Our Hospice team has an immediate opening for a part time Admissions/Visits/Liaison Nurse to be part of our expansion to the northern Virginia area.
 
Responsibilities: 
<ul>
<li>Perform initial assessment of patients' needs</li>
<li>Obtain fully informed consents before admitting patients to the hospice program</li>
<li>Serve as a clinical resource for referral sources to patients/family members</li>
<li>Facilitate end of life conversations and decision-making with the family or caregiver.</li>
<li>Prepare and discuss with physician an initial plan of care for patients</li>
<li>Establish working relationships that promote utilization of JSSA Hospice by hospitals, physicians, community-based case managers and other community agencies.</li>
<li>Represent our agency well when communicating with patients, families, physicians, and when participating in community network meetings.</li>
<li>Flexible work schedule - must be available Monday through Friday to complete up to 5 admissions a week.</li>
</ul>
Position Qualifications: 
<ul>
<li>An active RN license to practice in State of Virginia</li>
<li>A minimum of 3 years of acute care and/or hospice experience.</li>
<li>Demonstrated knowledge and skills necessary to provide meaningful care to and communicate with the terminally ill.</li>
<li>Excellent communication and customer service skills; attention to detail; strong organization skills.</li>
<li>Proficient with Microsoft Office 365 applications and the Homecare Homebase electronic medical record system.</li>
<li>A valid driver’s license, clean driving record and reliable/independent transportation to provide care to hospice patients throughout northern Virginia.</li>

Hospice RN - part time

Our Hospice in Fairfax has an immediate openings for an on call PRN nurse working Saturdays and Sundays from 8am-5pm providing patients and their families with exceptional end-of-life care that supports dignity and comfort for individuals, peace of mind to families, and honors cultural traditions and customs in the Fairfax, Loudoun, Prince William counties.
 
Functions: 
<ul>
<li>The nurse will provide information and admission visits as needed and deliver quality hospice and palliative care services to patients in their place of residence</li>
<li>Will perform initial assessment of patient’s needs, serve as a clinical resource for referral sources and patients/family members, facilitate end of life conversations and decision-making with the family or caregiver.</li>
<li>Will represent our organization when communicating with patients, families, and physicians.</li>
</ul>
Qualifications: 
<ul>
<li>An active State of Virginia RN license and a minimum of 3 years of acute care and/or hospice experience.</li>
<li>Demonstrated knowledge and skills necessary to provide meaningful care and to communicate with the terminally ill.</li>
<li>Excellent communication and customer service skills; attention to detail; strong organization skills, and proficient computer skills.</li>
<li>Proficiency with MS Office 365 and EMR systems. Familiarity with the Homecare Homebase electronic medical record a plus.</li>
<li>A valid driver’s license and reliable, independent transportation to provide care to hospice patients in Northern Virginia.</li>
<li>Must be available from 8am-5pm on call every Saturday and Sunday</li>

Hospice - RN on call weekends

Our Hospice team has immediate openings for full time On-Call Evening/Night Nurses to provide end of life care to patients in Northern Virginia: Fairfax, Loudoun, Prince William, City of Alexandria, and City of Arlington. Must be available for one of these two schedules:
 1. Weekday Schedule - Monday 5pm through Friday 8:30am
 2. Weekend Schedule - Friday 5pm through Monday 8:30am
 
Responsibilities: 
<ul>
<li>Provide routine and emergency care visits to patients as assigned and triaged by on call service</li>
<li>Make phone calls to assigned patients to triage symptoms</li>
<li>Provides follow up phone calls to assess efficacy of recommended treatments</li>
<li>Carries out full admissions when needed</li>
<li>Provide visits to confirm absence of vital signs unless families decline visits</li>
</ul>
