JSSA is looking for a full-time Nurse Practitioner to support our Montgomery County and Northern Virginia end of life patients and their families. Responsibilities: <ul><li>Clinical assignments are performed with minimal supervision.</li><li>Conduct comprehensive physical exams, evaluate eligibility for hospice, and monitor for disease progression and symptom changes.</li><li>Develop, implement, and update personalized care plans, focusing on comfort and aligning with patient/family goals.</li><li>Prescribe medications and therapies, manage side effects, and also provide non-pharmacological interventions for symptom relief.</li><li>Work closely with hospice physicians, nurses, social workers, chaplains, and bereavement counselors to ensure holistic care.</li><li>Maintain thorough records of assessments, evaluations, consultations, and patient progress. Document patient care in accordance with Agency policy and Medicare/Medicaid and other third-party requirements.</li><li>Perform home visits, facility visits, and sometimes inpatient evaluations.</li><li>Perform face to face visits in person and utilize telehealth on occasion (if law allows)</li><li>Assist with referral and admission process to review patient eligibility</li><li>Signs death certificates and MOLST/POLST forms as delegated by the hospice physicians.</li><li>Participate in the on call schedule which necessities a shared and equitable rotation to cover holidays, weeknights and weekends</li><li>Have sufficient knowledge of the Medicare Hospice Benefit and other third-party insurances including the ability to interpret these to the patient and family</li><li>Performs other appropriate duties as required</li></ul>Qualifications: <ul><li>Master's or Doctoral degree in Nursing from an accredited nurse practitioner program.</li><li>Advanced Certified Hospice and Palliative Nurse (ACHPN) highly preferred.</li><li>Minimum of 2 years of nurse practitioner experience, preferably in hospice, palliative care, geriatrics, or internal medicine.</li><li>License/Certification Active and unrestricted Maryland and Virginia NP License and NPI number required. Must be willing and able to make in person visits in both Maryland and Northern Virginia.</li><li>State and Federal controlled substance registration and DEA number required for prescribing.</li><li>Experience providing care in home and nursing homes/assisted living facilities.</li><li>Valid driver's license and reliable transportation to travel across Montgomery County and Northern Virginia as needed for visits.</li><li>Strong clinical assessment and critical thinking skills.</li><li>Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams.</li><li>Proficiency with Homecare Homebase or other hospice EMR systems preferred.</li><li>Knowledgeable in the Hospice Conditions of Participation and Hospice Eligibility as outlined in the Local Coverage Determination (LCD) Guidelines</li></ul>We Offer: <ul><li>Annual salary is $125,000.</li><li>Supportive work environment</li><li>Mileage reimbursement</li><li>Medical/Prescription/Dental/Vision insurance</li><li>403b Retirement Plan</li><li>Paid Leave (sick/vacation/personal/holidays)</li><li>And much more...</li></ul>The Jewish Social Service Agency (JSSA) has been helping people across the Washington metropolitan area meet emotional, social, and physical challenges for more than 120 years.
