Case Managers are responsible for supporting individuals with disabilities by providing clinical management and oversight of the professionals who work with them, as well as building a relationship and overseeing the families and individuals on their caseload.<br>
<br>
<b>REQUIRED EDUCATION AND EXPERIENCE</b><br>
<ul><li>Minimum of a Bachelor's Degree in Psychology, Social Work, or a related field</li><li>Minimum of 1-3 years experience working in the field of human services conducting intakes,assessments, and in-home services.</li><li>Consistent access to a reliable vehicle.</li><li>Willingness to travel or drive throughout the area to visit, perform quality checks, and provide services as needed.
</li><li>Must be a self-motivated and independent worker who can manage a flexible schedule.</li></ul><br>
<b>
SUMMARY OF RESPONSIBILITIES</b><br>
<ul><li>Conduct intakes: meet with families and/or individuals in an effort to explain the organization's services</li><li>Provide ongoing supervision and training to unlicensed direct care staff</li><li>Assign appropriately skilled Direct Support Professionals (DSPs) to ensure services are being provided at the highest quality.</li><li>Attend quarterly, yearly, or as-needed meetings for all clients on caseload.</li><li>Review staff monthly and quarterly notes for completion, and update as necessary.</li><li>Create and maintain clinically appropriate goals.</li><li>Work with families and internal supportive teams to initiate, coordinate, and facilitate, smooth positive transitions when individuals enter the program.</li><li>Work in partnership with colleagues, families, and individuals to establish positive, trusting relationships, and consistent clear communication.</li><li>Utilize effective problem solving skills and act as a model/mentor for others in demonstrating professional ethics, standards, and practices.</li><li>Supervise unlicensed staff.</li></ul><br>
