As a key member of our wound care team, you will: ➢ Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities➢ Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities➢ Lead the charge on wound assessment, treatment planning, and advanced procedures➢ Harness cutting-edge tech and evidence-based protocols to accelerate healing➢ Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and families➢ Keep your charting sharp and timely in our customized EMR➢ Help drive quality initiatives and support our mission of raising the bar in post-acute careQualifications: <ul><li>Active NJ Nurse Practitioner license (board-eligible not accepted) or Physician Assistant</li><li>CDS and DEA license (must obtain by end of training period)</li><li>At least 2 years of wound care experience</li><li>Previous experience in SNFs, long-term care, or rehabilitation facilities</li><li>Willingness to travel to designated partner facilities within a 40-mile radius of home</li><li>Comfortable with EMR, mobile, and laptop documentation</li><li>Strong interpersonal skills &mdash; collaborative, communicative, and adaptable</li></ul>If you&rsquo;re a hands-on Provider, join us in delivering exceptional wound care where patients need it most&mdash;at the bedside.Pay: $140,000.00 - $145,000.00 per yearBenefits:<ul><li>401(k)</li><li>401(k) matching</li><li>Dental insurance</li><li>Health insurance</li><li>Life insurance</li><li>Paid time off</li><li>Professional development assistance</li><li>Vision insurance</li></ul>Application Question(s):<ul><li>Are you able to travel within a 40-mile radius of your home, in the Passaic/Morris/Essex County area, to partner facilities to provide wound care?</li></ul>Experience:<ul><li>Skilled nursing facility: 1 year (Required)</li><li>Wound care: 1 year (Required)</li></ul>Work Location: In person
