JOB RESPONSIBILITIES:

Patient Care &amp; Clinical Services
<ul>
<li>Review and evaluate medication orders for accuracy, appropriateness, and potential interactions.</li>
<li>Monitor patient responses to prescribed drug therapies and make evidence-based recommendations.</li>
<li>Participate in ward rounds and clinical case discussions with medical teams.</li>
<li>Counsel patients and caregivers on the use of medications, side effects, and adherence.</li>
<li>Support medication reconciliation during patient admission, transfer, and discharge.</li>
</ul>
Drug Therapy Management
<ul>
<li>Monitor therapeutic drug levels and adjust dosages accordingly.</li>
<li>Implement pharmacovigilance protocols and report adverse drug reactions (ADRs).</li>
<li>Recommend appropriate drug alternatives based on patient condition, cost, and formulary.</li>
</ul>
Collaboration &amp; Communication
<ul>
<li>Serve as a resource for physicians and nurses regarding medication information.</li>
<li>Communicate any changes in medication regimens clearly to the healthcare team.</li>
<li>Coordinate with the pharmacy department for timely medication supply and formulary updates.</li>
</ul>
Medication Safety &amp; Compliance
<ul>
<li>Ensure compliance with national guidelines, hospital policies, and regulatory requirements.</li>
<li>Participate in drug audits, inventory checks, and quality improvement initiatives.</li>
<li>Promote antimicrobial stewardship and monitor antibiotic usage.</li>
</ul>
Documentation &amp; Reporting
<ul>
<li>Maintain accurate records of interventions, ADRs, and therapeutic outcomes.</li>
<li>Contribute to clinical documentation in patient medical records.</li>
<li>Assist in compiling data for hospital committees (e.g., Pharmacy &amp; Therapeutics Committee).</li>
</ul>
Training &amp; Education
<ul>
<li>Educate interns, pharmacy students, and hospital staff on drug-related topics.</li>
<li>Participate in continuing education and stay updated on recent advances in clinical pharmacy.</li>
<li>Any other work/responsibility assigned by the HOD/Management in the course of business/work whether within the same unit/dept or any other unit/dept from time to time.</li></ul></div></div></div></div>
