Responsibilities<ul><li>Function autonomously to care for patients with complex, acute, critical and chronic health conditions.</li></ul><ul><li>Prescribe medications including the frequency and dosage</li></ul><ul><li>Work individually and in interdisciplinary teams of resources and consultants</li></ul><ul><li>Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers</li></ul><ul><li>Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams</li></ul><ul><li>Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care</li></ul>Skills<ul><li>Demonstrated problem-solving and workflow management</li></ul><ul><li>High level of clinical proficiency and excellent decision-making skills.</li></ul><ul><li>Ability to work independently and as an effective member of a healthcare team.</li></ul><ul><li>Excellent oral and written communication skills.</li></ul><ul><li>Ability to effectively collaborate and build relationships with patients, families and all care providers</li></ul><ul><li>Proven organizational skills and ability to prioritize effectively.</li></ul><ul><li>Strong computer skills with knowledge and experience with Electronic Medical Records</li></ul>Qualifications<ul><li>Board Certified as a FNP</li></ul><ul><li>DEA Substance License required</li></ul><ul><li>BCLS/ACLS certification required</li></ul><ul><li>Prior APN experience preferred</li></ul>Job Type: Part-timePay: $120.00 - $180.00 per hourBenefits:<ul><li>Flexible schedule</li></ul>Medical Specialty:<ul><li>Emergency Medicine</li><li>Geriatrics</li><li>Internal Medicine</li><li>Primary Care</li><li>Psychiatry</li><li>Urgent Care</li></ul>Work Location: In person
