As a Lead Intensive Case Manager, you will assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services. This position mentors other Intensive Case Managers on the team and assists in case note reviews and approvals.

Job Details:
<ul><li>Full time, non-exempt position
</li><li>$24.00 per hour
</li></ul>Minimum Educational Qualifications : (one of the following)
<ul><li>Master&rsquo;s degree in social work, psychology, rehabilitation, activity therapies, counseling, or education and three years of direct care experience in mental health
</li><li>Bachelor&rsquo;s degree in nursing and three years of direct care experience in mental health
</li><li>Bachelor&rsquo;s degree in social work, sociology, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, or education; or be a registered nurse; and five years of direct care experience in mental health including two years of supervisory experience
</li></ul>Additional Minimum Requirements :
<ul><li>Effective communication and proficient computer skills
</li><li>Ability to resolve complex issues and respond effectively in crisis situations
</li><li>Valid driver&rsquo;s license with daily access to a privately maintained and insured vehicle
</li><li>Successful completion of the pre-employment process, including clearances and verifications

Mental Health - Lead Intensive Case Manager

Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer.
As a Registered Nurse, you will provide high quality, person-centered care to adults with intellectual and developmental disabilities. This position is responsible for providing medical oversight, training, and counseling for various medical diagnoses and monitoring and coordinating health care services for individuals in assigned areas. In this position, you will work in a hybrid environment, including remote work with travel necessary for on-site assessments.

</li><li>$38.50-39.50 per hour, may increase with nursing experience specific to intellectual and developmental disabilities

<ul><li>Registered Nurse with Connecticut licensure in good standing
</li><li>One year of nursing experience; intellectual and developmental disability experience preferred
</li><li>Effective communication and proficient computer skills

Keystone Human Services is seeking a full time Behavioral Health Professional to join our team in supporting adults with Intellectual and Developmental Disability. As a Behavioral Health Professional, you will promote person-centered care, develop behavioral support plans, and enhance the independence and well-being of the people we serve. 
 
$5000 Sign-on bonus as eligible per incentive guidelines . Behavioral Health Professionals earn $63,120 per year and have access to a competitive benefits package.

Key Responsibilities:
<ul><li>Support the development, implementation, and evaluation of individualized behavioral support plans
</li><li>Promote Person-Centered Planning and Positive Approaches to skill development
</li><li>Assist in designing teaching strategies that foster independence, responsibility, and autonomy
</li><li>Utilize effective data collection systems to measure outcomes and improve program effectiveness
</li><li>Train staff in treatment plan implementation and monitor clinical data for effectiveness
</li><li>Provide education and training on relevant clinical topics to enhance staff knowledge
</li><li>Collaborate with clinical teams to ensure ethical, evidence-based practices
</li><li>Participate in inter-agency coordination and foster constructive relationships with allied providers
</li><li>Maintain professional relationships with individuals served, their families, and team members
</li><li>Stay informed on human services regulations, behavior change techniques, and treatment of challenging behaviors or dual diagnoses
</li></ul>Qualifications:
<ul><li>Master&rsquo;s degree in Psychology, Special Education, Counseling, Social Work, Education, Applied Behavior Analysis, or Gerontology or Board Certified Behavior Analyst (BCBA)
</li><li>One year of postgraduate experience in behavioral treatment for individuals with IDD
</li><li>Ability to respond effectively in high-stress or critical situations
</li><li>Strong oral and written communication skills; ability to interpret and apply written information
</li><li>Intermediate computer skills, ability to use Microsoft Word, Excel, Outlook and learn web-based systems
</li><li>Valid driver&rsquo;s license and access to properly insured vehicle
</li><li>Successful completion of the pre-employment process, including clearances and verifications
