Please review the job description below and visit our website at www.lhcla.com for more information about Leading Home Care's mission.Responsibilities:<ul><li>Manage care for clients who receive in-home personal care services</li><li>Participate in Comprehensive Plan of Care (CPOC) planning meetings</li><li>Create and implement client-specific Care Plans based on the State CPOC and with input from the client or their representative, ensuring that clients&rsquo; medical, behavioral, and mental health needs are met</li><li>Supervise, train, direct, and evaluate Caregivers in regards to all aspects of client care, ensuring Care Plans are followed and that clients' health and well-being are maintained</li><li>Communicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services</li><li>Travel to clients' homes for meetings and supervisory visits quarterly, annually, and as needed to remain updated on client/family&rsquo;s satisfaction with services, health and well-being, living conditions, and progress toward goals</li><li>Conduct regular Supervisory Monthly Report / Quality Assurance phone calls with all clients, and other phone contacts as needed</li><li>Respond to critical incidents &amp; complete Critical Incident Reports (CIR) within mandated timeframe</li><li>Properly document all client communications, care plan or medical changes, visit notes, or incidents.</li><li>Maintain updated and accurate digital records for each client</li><li>Communicate with Director, Staffing Coordinators, and HR on a regular basis to discuss any needs or issues regarding clients or employees</li><li>Communicate with Finance Coordinator regarding Money Management services for specific clients</li><li>Follow directives related to Emergency Preparedness &amp; Response in dealing with natural or manmade disasters, assisting with client contacts and support as needed</li><li>Promote LHCL in the local community within allowed standards and establish positive and professional relationships with external stakeholders</li><li>Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed</li><li>Follow policies, procedures, and guidelines established by LHCL, Medicaid program manuals, and LDH-HCBS licensing standards</li><li>Model the LHCL Mission, Values, &amp; Leadership Behaviors</li><li>Additional tasks related to Client Care as needed</li></ul>Requirements:<ul><li>Bachelor's degree required, preferably in Human Services or related field</li><li>2 years' experience preferred (HCBS industry, case management, or other related experience)</li><li>Compassion and eagerness to serve elderly and disabled individuals</li><li>Strong management, communication and organization skills</li><li>Basic computer skills including Windows &amp; Microsoft Office</li><li>Able to operate office equipment</li><li>Basic addition/subtraction and general analytical skills</li><li>The ability to interact on a professional level with LHCL clients and their families, DSWs and representatives of other community-based organizations.</li><li>Valid driver's license, auto insurance, vehicle inspection, and reliable personal transportation; willingness to transport clients in personal vehicle</li><li>Must satisfactorily pass all required and applicable State &amp; Federal eligibility verifications including background, driver's license, and other required screeningsMust be able to read, write, and follow instructions in English</li><li>Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs.
