This is a virtual role open to candidates working from <b>Oregon or Washington</b><b>.</b> While the position is primarily virtual, there may be occasions when on‑site work is required at a designated Legacy Health location a minimum of four times a year.</p><p>
All new hires are required to travel to a Legacy Health office in Portland, Oregon prior to their start date to complete a new‑hire health assessment and new‑hire paperwork. Initial training and orientation may be conducted on‑site before transitioning to the remote schedule.</p>
Responsibilities:

Qualifications:

</b></div><ul><li>As required by licensure.
</li><li>BSN or degree in progress strongly preferred.
</li><li>BSN may be required for specific assignments.</li><li>
Experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.</li></ul><div><b>
Experience:
</b></div><ul><li>One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing residency program that is equivalent in content, curriculum and duration.</li><li>
Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized.</li></ul><div><b>
Skills:
</b></div><ul><li>Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community.
</li><li>Keyboard skills and ability to navigate electronic systems applicable to job functions.</li></ul>
Licensure:

Responsibilities:
<ul><li>Coordinates and facilitates interdisciplinary provision of comprehensive, patient-centered, quality health care throughout the continuum for patients with acute and chronic health conditions.</li><li>

Serves as an expert resource to the healthcare team regarding the continuum of care, efficient use of resources, Best Practice protocols, team-based care, quality indicators and improvements, and regulatory requirements.

</li><li>Partners with patients and families in identifying health care issues and barriers to self-care in order to set priorities and engage in appropriate interventions.

Qualifications:

Academic degree in nursing (BSN or higher) preferred.</li></ul><p><b>

This position requires extensive knowledge of disease management to include diagnostics, treatment and prognosis, community resources and healthcare reimbursement. Minimum 2 years clinical nursing experience required. Relevant experience in one or more of the following healthcare areas preferred:<ul><li>
Coordination of community resources</li><li>
Care management of diverse patient populations</li><li>
Ambulatory Care
</li></ul></li><li>Knowledge of levels of care throughout the health care continuum to include; inpatient, emergency care, rehab, home health, hospice, long term acute care, SNF, ICF, ALF with an overall understanding of utilization management and resource management.</li><li>
Working knowledge of Care Management models across the continuum.</li></ul><p><b>
Knowledge/Skills:</b></p><ul><li>
Knowledge of six core components of case management:<ul><li>

Case Manager - Care Management

Responsibilities:
<ul><li><div>Perform a variety of service-related activities supporting the Patient Transportation Program, including upkeep and maintenance of unit supplies, patient lifts, and other patient care tasks.</div></li><li><div>
Transport patients to ancillary departments as needed.</div></li><li><div>
Assist with patient care, including lifting/turning patients, orienting patients to room controls, preparing rooms for new admits, and transporting patient belongings.</div></li><li><div>
Document patient transport details in medical charts, including date, time, destination, mode of transportation, and signature.</div></li><li><div>
Confirm with RN that the patient is stable for transport before proceeding.</div></li><li><div>

Patient Transport Associate

Responsibilities:
<div>As a Respiratory Care Practitioner, you will perform a variety of duties including respiratory therapy, ventilator management, EKGs, and serving on the code team. Additional responsibilities may include exercise stress testing and pulmonary function testing. All duties are carried out in accordance with physician orders, using sound clinical judgment and technical expertise in patient care.</div><div></div><ul><li><b><br>
COMPETENCY:</b> Achieves and maintains advanced level clinical skills necessary to care for critical patients.
</li><li><b>PLANNING AND IMPLEMENTATION OF WORKLOAD:</b> Performs cardiopulmonary procedures using the knowledge, skills and abilities required for the age of patients (neonate, pediatric, adolescent, adult, and geriatric) served. Goals of the department are attained utilizing a clearly established daily plan of treatments, procedures, clean up and maintenance with cooperation through all shifts regardless of assignments or job title.</li><li><b>
DOCUMENTATION</b><b>:</b> Complete, accurate and timely information is gathered and documented in the department and patient records. Accurate charges and record keeping is done to ensure proper billing.</li><li><b>
COMMUNICATION</b><b>:</b> Verbal and non-verbal age-appropriate communications (neonate, pediatric, adolescent, adult and geriatric) are open and positive with peers, supervisors, physicians, patients and all outside customers, under stressful and non-stressful circumstances. Patients and co-workers&rsquo; confidentiality is maintained.</li><li><b>
ENVIRONMENT/SAFETY</b><b>:</b> Clinic environment, equipment, and personal workspace are maintained in a neat and safe manner. Annual safety and other hospital wide educational requirements are met in a timely fashion and clearly documented.</li><li><b>
PROFESSIONAL DEVELOPMENT</b><b>:</b> Professional education is advanced through regular participation in internal/external education opportunities in both broad and discipline-specific topics. Completes annual competencies and keeps accurate files in personal portfolio.</li><li><b>
