What You Bring</b></div><div><ul><li>
Bachelor&rsquo;s degree in human services, Psychology, Social Work, Sociology, or related field preferred</li><li>
HS Diploma with 12 credits in Social Sciences and 2 years&rsquo; experience accepted</li><li>
Strong computer skills: EHR experience preferred</li><li>
Valid Driver&rsquo;s License and ability to travel within Bucks County</li><li>
Strong verbal and written communication skills</li><li>
Ability to meet the physical demands of the role, with or without accommodation</li></ul></div><p></p><div><b><br>
Your Role</b></div><div><ul><li>
Provide short-term case management for individuals seeking services at LVF</li><li>
Partner with clients in making connections to community resources including accessing health insurance, housing programs, county assistance benefits, transportation options, and providing support in problem solving benefit issues</li><li>
Travel to other LVF sites to help with Medical Assistance applications</li><li>
Receive referrals and provide immediate follow-up</li><li>
Document all work in our electronic health system</li></ul></div><div></div><div><b><br>
