COPD Inpatient Educator/Pulmonary Rehabilitation 


Position Description

The COPD Educator/Pulmonary Rehabilitation position is responsible for educating and assessing patients diagnosed with COPD in hospital. They will review the patient's history and care plan, educate the patient on the disease process, as well as connect them with resources both inside and outside of the hospital in order to help with the management of their disease and improve their quality of life. The COPD educator/Pulmonary Rehabilitation position is also responsible for reviewing referrals and identifying candidates who qualify for pulmonary rehabilitation. They will contact patients to explain the program and help to schedule them for their appointments as well as coordinating any needs they may have that act as barriers to participating. They will be able to support the pulmonary rehabilitation staff and be trained both in pulmonary rehabilitation as well as six-minute walk testing. 


Daily Tasks:
<ul>
<li>Identify COPD inpatients from generated list and COPD consults and review care plan and history</li>
<li>Visit, assess and provide COPD education to inpatients on list</li>
<li>Identify and discuss recommendations of changes to treatment plan with hospitalist and PCP</li>
<li>Generate referrals to outpatient resources including pulmonology, pulmonary rehabilitation, community case management, palliative care</li>
<li>Documenting COPD consult in EMR</li>
<li>Review referrals for pulmonary rehabilitation and identify qualified candidates for pulmonary rehabilitation</li>
<li>Contact referrals and explain pulmonary rehabilitation program. Identify any barriers and connect patients to resources if necessary.</li>
<li>Co-ordinate with pulmonary rehab staff in scheduling initial pulmonary rehab visits and mail out initial packets to patients</li>
<li>Cross-trained for 6 minute walks and pulmonary rehabilitation and will support pulmonary rehabilitation staff</li>
<li>Help out in respiratory therapy staff as needed</li>
</ul>
Monthly Tasks:
<ul><li>Compile documentation of COPD education done for the month and report COPD dashboard to manager</li>
<li>Will attend Respiratory Therapy department meetings (bi-monthly)</li>
</ul>
Annual tasks:
<ul>
<li>Maintain Current Respiratory Care license</li>
<li>Evaluation with manager</li>
<li>Complete Webed course and other required education</li>
<li>Maintain current NRP (every other year)</li>
<li>Maintain annual RQI cognitive skills (BLS, PALS, ACLS)</li>
<li>Ensure annual staff competencies are completed (both department level and hospital wide)</li>
<li>LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.</li></ul></div>

 Position Summary
 
 
 
 Under assessment and supervision of the Registered Nurse and according to established policies and procedures, performs patient care activities in order to assist health care providers in performing various clinical and technical procedures in the patient care area.
 
 
 
 Responsibilities

 Complete assigned patient care tasks
 </li>
 <li>
 Accurately collect, perform and document urine pregnancy tests and quality controls associated with it.
 </li>
 <li>
 Performing one-to-one monitoring on all restrained, suicidal, combative and other high-risk patients when requested by RN or physician.
 </li>
 <li>
 Obtain vital signs and weights
 </li>
 <li>
 Insert Foley catheter aseptically.
 </li>
 <li>
 Collect, label, and accurately enter specimens obtained.
 </li>
 <li>
 Lifts, transfers, transport patients
 </li>
 <li>
 Enters work orders in computer
 </li>
 <li>
 Stocks supplies at bedside and puts supplies away prior to leaving shift
 </li>
 <li>
 Documentation in the electronic medical record
 
 
 </li>
 </ul>
 
 Requirements

 <li>
 Minimum requirement, high school diploma or GED.
 </li>
 <li>
 Read, write, and follow verbal or written instructions
 </li>
 <li>
 Electronic medical record competency
 </li>
 <li>
 Maintain CPR certification.
 </li>
 <li>
 Work independently and professionally and treats customers and employees with respect
 </li>
 <li>
 Obtain/maintain 12 lead ECG testing proficiency
 </li>
 <li>
 Obtain/maintain phlebotomy skill.
 </li>
 <li>
 Obtain/maintain Ortho-Glass skills
 </li>
 <li>
 This position will be required to work in the Emergency Department as needed
 </li>

Job requires knowledge, skill and ability to provide basic care for a neo-natal, pediatric, adolescent, adult and geriatric patient population
 
 
 
 Responsibilities

 Completes patient intake and discharge under supervision of a physician, NP, and/or RN/LPN. Completes appropriate documentation in the medical record (paper and EMR).

 Performs technical tasks, as authorized : checks vital signs, collects routine specimen, performs ECG, phlebotomy,

 Assists providers with procedures.
 </li>
 <li>
 Responds to emergency situations in a prompt and precise manner, and assists other health care providers as needed
 </li>
 <li>
 Informs Physician, NP, or RN/LPN of patient's symptoms, reactions, progress and other information pertaining to patient's care. Documents information in the medical record (paper and electronic) according to policies and procedures.
 </li>
 <li>
 Presents a positive appearance and attitude demonstrating excellent guest relation skills, interpersonal skills and team values within the Professional Corporation, Health System and community.
 </li>

 Observes and maintains patient, staff and corporate confidentiality per policies and procedures.

 Requirements

 Demonstrates ability to read, write, follow verbal or written instructions and perform simple mathematical calculations.

 A high school diploma or equivalent is required.
 </li>

 AAMA or AMT Medical Assistant certification preferred upon hire and required within six months of hire date.
 </li>
 <li>
 Maintains BCLS certification
 </li>
 <li>
 Completes orientation as specified to acquire the necessary familiarity with LMHS policies, procedures and patient care techniques within assigned patient care areas.
 </li>
 <li>
 Demonstrates ability to assist in the care for pediatric, adolescent, adult and geriatric patients within the scope of the job description while under the supervision of a licensed physician, NP and/or RN/LPN.
 </li>
 <li>
 Demonstrate knowledge, skill and ability to perform: phlebotomy, ECG, vital signs, ear lavage, monofilament, glucometer, occult blood, visual and auditory acuity, strep screen, urinalysis, urine pregnancy, specimen collection and preparation as applicable to the practice setting.
 </li>
 <li>
 Competency with personal computer usage and medical record documentation
 </li>
 <li>
