Life Care Centers of America facilities operate a Skilled Nursing Facilities (SNFs) that provide:
 
<ul>
<li>Long term care for residents who require ongoing skilled support and compassionate daily assistance</li>
<li>Short term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury</li>
<li>Collaborative care through an interdisciplinary team approach with nursing, therapy, and medical staff working together</li>
<li>A diverse patient population including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs</li>
</ul>
Patient mix varies slightly by facility. As a therapy professional, you will help patients achieve functional progress and improve quality of life within a supportive, patient-centered environment.
 
Position Summary: 
The Transportation Coordinator/Driver loads, transports, and unloads patients for medical appointments and activity outings in accordance with all applicable laws, regulations, and Life Care standards.
 
Education, Experience, and Licensure Requirements: 
<ul>
<li>High school graduate or equivalent</li>
<li>Valid driver’s license or valid commercial driver’s license in current State, as applicable, with a satisfactory driving record per Life Care standards</li>
<li>Prior experience in health care or geriatrics preferred</li>
</ul>
Specific Job Requirements: 
<ul>
<li>Complete pre inspections before use of vehicle</li>
<li>Monitor regular maintenance (oil changes, tire rotations, etc.)</li>
<li>Make independent decisions when circumstances warrant such action</li>
<li>Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility</li>
<li>Implement and interpret the programs, goals, objectives, policies, and procedures of the department</li>
<li>Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation</li>
<li>Maintains professional working relationships with all associates, vendors, etc.</li>
<li>Maintains confidentiality of all proprietary and/or confidential information</li>
<li>Understand and follow company policies including harassment and compliance procedures</li>
<li>Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training</li>
</ul>
Essential Functions:

<li>Safely load, transport, and unload patients</li>
<li>Exhibit excellent customer service and a positive attitude towards patients</li>
<li>Assist in the evacuation of patients</li>
<li>Demonstrate dependable, regular attendance</li>
<li>Concentrate and use reasoning skills and good judgment</li>
<li>Communicate and function productively on an interdisciplinary team</li>
<li>Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours</li>
<li>Read, write, speak, and understand the English language</li>
