DSPThe Direct Support Professional will provide services to seniors and adults with disabilities in an innovative, community based home environment located in Murfreesboro. There will be four residents per home which will provide for a low staff to resident ratio.Job Duties:&middot; Provide support and assistance to residents in a variety of areas (domestic, personal hygiene, work, social and recreational skills, self-help skills)&middot; Provide coordination of outpatient care for residents in a variety of areas (primary care, specialty care, dental, hearing, vision)&middot; Observe and report changes to residents&rsquo; health and behaviors&middot; Arrange and actively participate in residents&rsquo; activities including holidays, day trips, and seasonal events&middot; Prompt and assist residents with activities of daily living&middot; Assist with meal planning and preparation&middot; Provide services to residents based on the Plan of Care&middot; Ensure all services are provided in accordance with licensure and BHO standards&middot; Ensure home and grounds are clean and orderly at all times&middot; Collaborate with the Housing Manager as required&middot; Thoroughly document progress notes&middot; Collaborate with all stakeholders to develop Plans of Care&middot; Report and review all resident medical concerns with the nursing staff&middot; Collaborate with Housing Manager on quality of care issues&middot; Report and review all incident reports with the Housing Manager and Director&middot; Maintain compliance with all agency policies&middot; Other duties as appropriate and as needed that are essential to meet licensure and BHO requirementsJob Requirements&middot; Medical Assistant, CNA or CNT preferred, but applicants with caregiving experience for seniors or adults with disabilities will be considered&middot; High School Diploma or GED&middot; Valid Tennessee Driver License&middot; Must pass Nationwide Criminal Background Check&middot; Experience with TennCare Choices Program preferred but not requiredWork Remotely<ul><li>No</li></ul>Job Types: Full-time, Part-timePay: $18.00 - $19.00 per hourBenefits:<ul><li>401(k) matching</li><li>Health insurance</li></ul>Work Location: In person
