Position Summary:
Location: Sunrise Hospital
Schedule: Monday - Friday 10:00 am - 7:00 pm
The GIP RN for Infinity Hospice Care (IHC), works primarily in hospitals where IHC holds a current contract for hospital GIP and supports the hospital inpatient palliative care service. This GIP RN gains a high understanding of the IHC intake process and support transitions to both home IHC and NEVPC.
The GIP RN promotes the hospice program, enrolls patients, and supports providers when needed to transition patients to IHC and NEVPC. The GIP RN engages patients, families, caregivers, and updates hospital staff as needed. The GIP RN acts as a liaison in supporting documentation, notes, and electronic information transmission to both IHC and NEVPC.
This position reports directly to the Director of Patient Care Services (DPS) of IHC as well as any assigned hospital contracted Providers and Advance Practice Nurses.
Requirements: 
Qualifications:<ul><li>
Active nursing license from an accredited program and in good standing with the 
State Board of Nursing in the state of licensure</li><li>
Strong hospice knowledge 
</li><li>
Outgoing personality, team player and maintain confidentiality 
</li><li>
Ability to prioritize and be flexible 
</li><li>
Proven success in healthcare marketing preferred 
</li><li>
Must maintain a valid driver&rsquo;s license and current automobile insurance</li></ul>
Responsibilities: 
<ul><li>
Can perform hospice admission for both hospital GIP and start the admission process for patients transitioning to home setting. 
</li><li>
Ensures an excellent hospital to home hand off. 
</li><li>
Works with social services and/or placement when most appropriate care setting is 
difficult to determine. 
</li><li>
Utilize marketing material, discuss key programs and benefits of IHC and NEVPC 
with hospital patient and family 
</li><li>
Use and promote referral screening forms and assist in presentations to case 
management, facility stakeholders, and families 
</li><li>Assist in gathering information to expedite hospital discharge to SNFs, help resolve issues and assist in placement.

Understands IHC inpatient admission criteria.

Physical Therapist
General Summary:
Provide coordinated care to clients of all age groups. Plans, implements and evaluates client care plans to restore or maintain patient well-being. Plans organize and conducts physical therapy treatment based on the medical referral and their evaluation.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:<ul><li>
Promote /exemplify Company mission, vision and values at all times.</li><li>
Perform clinical assessments; identifies patient needs and appropriateness of client.</li><li>
Document, prepare/revise and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.</li><li>
Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).</li><li>
Plan and provide appropriate treatment for neuro-muscular neurological and orthopedic disorders based on medical referral and therapy assessment.</li><li>
Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment.</li><li>
Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference/Team Meetings.</li><li>
Monitors assigned cases to ensure compliance with the requirements of third-party payers.</li><li>
Completes appropriate documentation in a timely manner to assure compliance with company policy.</li><li>
Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.</li><li>
Meet mandatory continuing education requirements of the Agency/licensing board.</li></ul>

Requirements:

Experience:<ul><li>
Two (2) years experience as a PT in a clinical care setting.</li><li>
Preferred, Home health experience.</li></ul>
Skills:<ul><li>
Therapy skills as defined as generally accepted standards of practice.</li><li>
Excellent interpersonal skills and ability to communicate effectively.</li><li>
Demonstrates proven decision-making skills.</li><li>
If required to make visits, Hepatitis profile.</li><li>
Must read, write and comprehend English.</li></ul>
Education:<ul><li>
Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.</li></ul>
Licensure/Certification:<ul><li>
Current driver&rsquo;s license in good standing. It is the responsibility of the employee to renew their driver&rsquo;s license before it expires in order to continue employment.</li><li>
Reliable transportation and auto liability insurance.</li><li>
Current State professional license as a PT in any/all applicable states.</li><li>
Current CPR</li></ul>
On-going employment as a PT always requires maintenance of a valid PT license in the state which the therapist practices in and a valid CPR certificate. It is the responsibility of the employee to renew these before they expire in order to continue employment.

Prolonged standing/walking required.</li><li>
The ability to lift/transfer up to 50 lbs.</li><li>
Ability to meet deadlines and patient needs, make quick decisions and allocate resources under stressful conditions.</li><li>
Meet patient/family individualized psycho social needs.</li><li>
Requires hand-eye coordination and manual dexterity.</li></ul>
Environmental/Working Conditions:<ul><li>
Works in patient&rsquo;s home in various conditions.</li><li>
Possible exposure to blood borne pathogens, bodily fluids and infectious diseases.</li><li>
Some exposure to unpleasant weather; PRN emergency call.</li><li>
