Essential Functions:<ul><li>
Assist clients with tasks within the scope of a home health aide based on the Plan of Care (POC).</li><li>
Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.</li><li>
Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.</li><li>
Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.</li><li>
Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.</li><li>
Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.</li><li>
Meet mandatory continuing education requirements of the Agency/licensing board.</li></ul>

Requirements:

Experience:<ul><li>
(1) year experience as a home health aide or certified nursing assistant in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.</li></ul>
Skills:<ul><li>
Ability to establish and maintain effective working relationships with all segments of the staff, and the Agency leadership team.</li><li>
Shall be able to read, write and comprehend English.</li></ul>
Education:<ul><li>
High School Graduate or equivalent</li></ul>
Licensure/Certification:<ul><li>
Home Health Aide Certificate or other similar certification held in good standing. Certification requirements may vary by state and/or participating program.</li><li>
A formal certificate of completion for an HHA/CNA training program that meets or exceeds standards established by Health and Human Services. Completion of all in-service hours per year. Ongoing requirements may vary by state and/or participating program.</li><li>
Current CPR certification.</li><li>
Current driver&rsquo;s license in good standing. The employee is responsible for renewing their driver&rsquo;s license before it expires to continue employment.</li><li>
Reliable transportation and auto liability insurance.</li><li>
On-going employment as a HHA always requires maintaining a valid HHA certificate in the state where the caregiver practices in and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.</li></ul>

The ability to lift/transfer up to 50 lbs.</li><li>

<div>Description:

General Position Description:
The Admission Nurse is a Registered Nurse responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, documentation, and direct nursing care within the policies and procedures of the Agency. The Admission Nurse is responsible for accurately completing the OASIS assessment, administering skilled nursing care to patients, and determining what additional disciplines and/or programs are required for the patient's care. The Admission Nurse makes the evaluation and assessment visits for an assigned geographic region, which may include the entire agency location, using a point-of-care electronic documentation platform. The Admission Nurse will be required to educate staff and deliver training programs within an assigned geographic region.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:<ul><li>
Make initial evaluation visits as applicable and consult the attending physician to establish the care plan. Responsible for timely communication of the care plan to the RN case manager, other members of the interdisciplinary team, and the clinical manager. Complete SOC OASIS and develop an appropriate plan of care based on physician orders using an electronic point of care documentation system. Submits documentation in a timely and complete manner according to agency policy</li><li>
They may be required to provide clinicians with training and follow-up assistance on plan of care development, appropriate OASIS documentation, and utilization of clinical specialty programs. They may also be required to do Recertification visits and resume-of-care visits/oasis assessments.</li><li>
Required to participate in the On-Call rotation of an assigned geographic region.</li><li>
Participates in continuing education r/t OASIS completion and review, ICD-10 coding and industry standards and trends as directed by the Administrator.</li><li>
A minimum of 6 hours of ICD-10 coding-related education is required annually. (Agency reimbursed)</li><li>
Productively, the expectation is 3-4 oasis visits per day, depending on availability and geography.</li><li>
May be required to perform other skilled nursing visits as needed.</li><li>
Meet mandatory continuing education requirements of the agency/licensing board.</li></ul>

General Summary: The Clinical Manager maintains the quality of client care in accordance with the Agency&rsquo;s objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided. This position also directs and manages the field staff of the Agency while coordinating care with the interdisciplinary team, patient/family and referring Agency.

Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.</li><li>
Participate in community related functions as necessary.</li><li>
Assist with the orientation of new professional staff to familiarize them with Agency policies and procedures.</li><li>
Take client visits in the home setting as needed.</li><li>
Participate in audits, U.R. interdisciplinary team conferences and represents Agency in the community as requested.</li><li>
Review each clinician&rsquo;s schedule for recertification, discharges, and aide supervisory visits.</li><li>
Review weekly schedule submitted by clinician for changes or updates.</li><li>
Perform other duties as directed by Area DOCS/ DOCS.
