General Position Description:
The Admission Nurse is a Registered Nurse responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, documentation, and direct nursing care within the policies and procedures of the Agency. The Admission Nurse is responsible for accurately completing the OASIS assessment, administering skilled nursing care to patients, and determining what additional disciplines and/or programs are required for the patient's care. The Admission Nurse makes the evaluation and assessment visits for an assigned geographic region, which may include the entire agency location, using a point-of-care electronic documentation platform. The Admission Nurse will be required to educate staff and deliver training programs within an assigned geographic region.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:<ul><li>
Make initial evaluation visits as applicable and consult the attending physician to establish the care plan. Responsible for timely communication of the care plan to the RN case manager, other members of the interdisciplinary team, and the clinical manager. Complete SOC OASIS and develop an appropriate plan of care based on physician orders using an electronic point of care documentation system. Submits documentation in a timely and complete manner according to agency policy</li><li>
They may be required to provide clinicians with training and follow-up assistance on plan of care development, appropriate OASIS documentation, and utilization of clinical specialty programs. They may also be required to do Recertification visits and resume-of-care visits/oasis assessments.</li><li>
Required to participate in the On-Call rotation of an assigned geographic region.</li><li>
Participates in continuing education r/t OASIS completion and review, ICD-10 coding and industry standards and trends as directed by the Administrator.</li><li>
A minimum of 6 hours of ICD-10 coding-related education is required annually. (Agency reimbursed)</li><li>
Productively, the expectation is 3-4 oasis visits per day, depending on availability and geography.</li><li>
May be required to perform other skilled nursing visits as needed.</li><li>
Meet mandatory continuing education requirements of the agency/licensing board.</li></ul>

Experience:<ul><li>
A minimum of three (3) years of professional experience with at least one (1) year of home care experience is required.</li><li>
One year of experience with an electronic medical record</li></ul>
Skills:<ul><li>
Excellent knowledge of Federal and State regulations about home health.</li><li>
Excellent knowledge of home care standards and practices.</li><li>
Ability to multitask with strong problem-solving skills</li><li>
Excellent written and verbal skills</li><li>
Strong customer service skills</li><li>
Ability to travel as required to perform job functions</li></ul>
Education:<ul><li>
Completion of an accredited Registered Nursing Program.</li><li>
Preferred, BSN.</li></ul>
Licensure/Certification:<ul><li>
Current driver&rsquo;s license in good standing. The employee is responsible for renewing their driver&rsquo;s license before it expires to continue employment.</li><li>
Reliable transportation and auto liability insurance.</li><li>
Current State professional license as an RN in any/all applicable states, BSN preferred</li><li>
OASIS and/or ICD-10 coding certification preferred.</li><li>
Current CPR.</li><li>
If not certified, one must be willing to become OASIS certified (at the company&rsquo;s expense) within 6 months of being hired in the position.</li></ul>
Ongoing employment as an RN always requires maintenance of a valid RN license in the state where the nurse practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.

General Summary: The Clinical Manager maintains the quality of client care in accordance with the Agency&rsquo;s objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided. This position also directs and manages the field staff of the Agency while coordinating care with the interdisciplinary team, patient/family and referring Agency.
Patient Population: All patients assigned to the provider numbers in the employee&rsquo;s area of responsibility.
Essential Functions:<ul><li>

Coordinate and schedule the daily work load of the office while utilizing Agency personnel to their fullest capacity.</li><li>
Provide daily supervision of field nurses and Home Health Aides to ensure the quality of services and maintenance of standards.</li><li>
Review and investigate the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients.</li><li>
Direct staff toward the appropriate use of community resources.</li><li>
Appraise staff performance during their probationary period and annually with the Director of Clinical Services, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.</li><li>
Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.</li><li>
Participate in community related functions as necessary.</li><li>
Assist with the orientation of new professional staff to familiarize them with Agency policies and procedures.</li><li>
Take client visits in the home setting as needed.</li><li>
Participate in audits, U.R. interdisciplinary team conferences and represents Agency in the community as requested.</li><li>
Review each clinician&rsquo;s schedule for recertification, discharges, and aide supervisory visits.</li><li>
Review weekly schedule submitted by clinician for changes or updates.</li><li>
Perform other duties as directed by Area DOCS/ DOCS.
</li><li>May be requested by Area DOCS/ DOCS to backup other clinical staff when necessary.</li></ul>

Requirements:

Minimum 2 years of experience as a registered nurse in a Home Health Agency.</li><li>

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.

Physical Requirements:<ul><li>
Prolonged sitting required in an office environment which involves minimal and occasional lifting of medical supplies and charts.</li><li>
If visiting patients, prolonged standing and walking required, with the ability to bend, stoop, reach and move client weight up to 250 pounds as well as carry up to 30 pounds.</li><li>
The ability to access clients&rsquo; homes which may not be routinely wheelchair accessible is required.</li><li>
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client&rsquo;s condition and to perform client care.</li><li>
Ability to handle stressful situations in a calm and courteous manner at all times.</li><li>
Requires working under some stressful conditions to meet deadlines and Agency needs.</li></ul>

Physical Therapist
General Summary:
Provide coordinated care to clients of all age groups. Plans, implements and evaluates client care plans to restore or maintain patient well-being. Plans organize and conducts physical therapy treatment based on the medical referral and their evaluation.

Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference/Team Meetings.</li><li>
Monitors assigned cases to ensure compliance with the requirements of third-party payers.</li><li>
Completes appropriate documentation in a timely manner to assure compliance with company policy.</li><li>
Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.</li><li>
