POSITION: Case Manager DEPARTMENT: Support Services REPORTS TO: Support Services Manager STATUS: Non-Exempt, HourlyCompany Overview LivWell Community Health Services is a non-profit organization providing comprehensive health and support services, education, and advocacy, as related to HIV/AIDS, sexually transmitted diseases, and viral hepatitis. Quality health care is provided to persons living with HIV/AIDS or sexually transmitted diseases, support services are provided to HIV clients and their families, and prevention education and testing are provided to all clients, their families, and the community at large.Position Overview The Case Manager offers person-centered support while guiding clients through the social services and healthcare systems. The Case Manager works closely with clients to provide coordination of resources, goal-setting, and establish continuity of care through ongoing evaluation of needs and personal support systems. This position also collaborates with a multidisciplinary team to identify and eliminate barriers to care and treatment adherence.As a Case Manager, typical duties would be:<ul><li>Meeting with clients to conduct a comprehensive assessment of needs, verify eligibility for services, and introduce opportunities within the program for assistance</li><li>Completing client enrollment into eligible programs and developing an Individualized Care Plan in collaboration with the client to secure suitable support services and assistance in alignment with the program guidelines</li><li>Determining and providing the level of assistance as deemed by the support level, and referring emergency needs to the appropriate service area</li><li>Arranging client-centered services in order to coordinate care plans, such as area resources, transportation to appointments, education, advocacy, and linking to external agencies for needed services</li><li>Making regular contact with clients via phone, text, in-person, home visits, or other means to assess needs and record progress toward goals</li><li>Creating files, inputting data, and documenting encounters appropriately</li><li>Collaborating with a multidisciplinary team to assist in client medication adherence efforts and report status to the team</li><li>Assisting with the development of the orientation process for new clients and facilitating such processes</li><li>Managing the scheduling and completion of the recertification process</li><li>Working collaboratively with external partners and funding agencies to establish a network of available external client resources</li><li>Assist in maintaining a directory of available support services providers within LivWell&rsquo;s service area</li><li>Ensuring compliance with quality standards, adhering to departmental goals, objectives, standards of performance, and all policies and procedures</li><li>May assist clients with Medicaid, Medicare, insurance, and disability documentation</li><li>Performing other duties as assigned and occasional travel as required for training, in-home visits, and clinics</li></ul>This job may be for you if:<ul><li>You have well-developed active listening skills</li><li>You can work under pressure</li><li>You see the benefit of working with diverse populations</li><li>You can patiently work with others to encourage, counsel, and educate them to develop goals and plans for achievement</li><li>You are able to balance competing priorities</li><li>You pay attention to details</li><li>You are a tactful and clear communicator, both in written and verbal form</li><li>You understand the importance of privacy and staying in compliance with regulations</li><li>You are comfortable working both independently and collaboratively</li><li>You can accommodate occasional evening and weekend work hours plus travel, including overnight and multiple days, which may be solo travel.</li><li>You are willing to become a certified HIV tester to assist with events as necessary</li><li>You can lift files, documents, supplies, etc., up to 25 pounds</li><li>You care about the mission and want to make an impact</li></ul>Required Qualifications<ul><li>Bachelor&rsquo;s degree in nursing, social work, counseling, psychology, human services OR possess a Kentucky state license as a licensed practical nurse (LPN) OR an Associate Degree in Nursing (ADN)</li><li>Knowledge of community resources and sensitivity towards people living with HIV/AIDS</li><li>Valid driver&rsquo;s license and are insurable to drive LivWell CHS&rsquo; vehicles</li><li>Experience with basic technology and office computer programs, such as Microsoft Office Suite</li></ul>Preferred Qualifications<ul><li>Two years&rsquo; experience in social work or a human service-related field</li><li>Bilingual</li></ul>Physical Demands:The physical demands described here represent those that any worker must meet to perform the essential functions of this job successfully.
