Our team strives to maintain a positive, enthusiastic environment aimed at helping individuals achieve their maximum potential.Minimum Qualifications:The Case Manager position requires a bachelor&rsquo;s degree in a human service related field including but not limited to psychology, education, rehabilitation counseling or counseling from an accredited institution; and one year of paid experience in a human services field providing direct consumer services or case management or a bachelors or masters degree in social work from a social work program accredited by the Council on Social Work Education.Thirty hours of graduate level course credit in a human service-related field may be substituted for the one year of required paid experience. Experience may be obtained before or after completion of the degree or licensure.This position requires reasonable ability to interact with persons outside the agency. The Case Manager must be flexible and capable of working with a variety of people. Case Manager must also have good computer skills, decision making skills, ability to solve problems, make decisions, resolve conflicts, ability to deal calmly in crisis situations, strong interpersonal skills with ability to be compassionate and firm and always maintain confidentiality, flexibility and professionalism.EPSDT (Early and Periodic Screening, Diagnostic and Treatment) experience preferred. Function: The Case Manager position provides services through implementation of Home and Community Based services/programs to eligible recipients. Living Water Family Services Case Managers assures that specific DHH guidelines are being met and provide extensive case work to foster transitions from institutions to the home and community.Principle Responsibilities:The Case Manager is responsible for the following:&middot; Identifying, compiling and reviewing any necessary assessments and other documents that relevant to the recipient/family&rsquo;s needs (including health needs), interests, strength, preferences and desired outcomes.&middot; Assist in arranging professional evaluations and other components of the assessment to develop strategies for obtaining the services, (including EPSDT, PCS services and making referrals for KIDMED), resources and supports needed to achieve the recipient&rsquo;s personal outcomes.&middot; Ensure recipient/family choice,&middot; Follow up with the recipient/family on a regular basis but no less than quarterly. At the quarterly face-to-face visit discuss with recipient/family if all services are appropriate and satisfactory and being delivered as planned or if changes are required.&middot; Assist the recipient/family to use the findings of formal and informal assessments to develop and implement support strategies to achieve the personal outcomes defined and prioritized by the recipient/family in the approved CPOC.&middot; Have knowledge of all Medicaid services and informal/natural resources, that may be useful in developing strategies to support the recipient in attaining his/her desired personal outcomes.&middot; Assist and remove any barriers that the EPSDT recipient/family may have in obtaining the identified services.&middot; Advocate on behalf of the recipient/family to help establish, expand, maintain and strengthen the natural support network, and obtain appropriate services. This may involve calling and/or visiting recipient/family, community groups, organizations or agencies with or on behalf of the recipient/family.&middot; Inform the recipient/family no less than quarterly of all Medicaid services for which they maybe eligible.&middot; Prepare the annual CPOC and appropriate updates, assessments.&middot; Document in progress notes all discussion regarding Medicaid services.&middot; Keep the required documentation in the record.Job position requires travel, applicant must have reliable transportation.Job Type: Full-timePay: From $18.00 per hourBenefits:<ul><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Work Location: In person
