We are seeking a compassionate, dedicated, and professional <b>Registered Nurse (RN)</b> to join our Home Health and Hospice team. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while promoting patient-centered care and interdisciplinary collaboration.<br>

<b>Key Responsibilities:</b><br>
</div>
<ul><li><br>
Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.</li>
<li>Develop individualized care plans in collaboration with physicians, patients, and families.</li>
<li>Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered.</li>
<li>Monitor, record, and report patient progress and changes in condition to physicians and other care team members.</li>
<li>Provide education to patients and families regarding disease management, medications, and home care techniques.</li>
<li>Coordinate patient care with interdisciplinary teams, including therapists, social workers, and home health aides.</li>
<li>Ensure accurate and timely documentation in the Electronic Medical Record (EMR).</li>
<li>Participate in on-call rotation as assigned.</li>
<li>Maintain compliance with all state, federal, and agency policies, procedures, and regulations, including HIPAA.</li>
Provide compassionate end-of-life care and emotional support to patients and families in hospice settings.<br>

<b>Qualifications:</b><br>
</div>
<div><br>
<b>Education &amp; Licensure:</b><br>
</div>
<ul><li><br>
Current and valid <b>Registered Nurse (RN)</b> license in the <b>State of Texas</b> (or compact state).</li>
<li>Graduate of an accredited <b>School of Nursing (BSN or ADN)</b>.</li>
<li>Current <b>CPR (BLS)</b> certification required.</li>
<li>Valid <b>Driver&rsquo;s License</b>, <b>auto insurance</b>, and reliable transportation required.<br>
</li>
</ul><div><br>
<b>Experience:</b><br>
</div>
<ul><li><br>
Minimum <b>one (1) year of clinical nursing experience</b> preferred (home health or hospice experience strongly preferred).</li>
<li>Knowledge of OASIS and EMR charting preferred for home health nurses.</li>
Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols.<br>
