Livingston Memorial is looking for a Full Time Registered Nurse, 32-40 hours a week to support our Home Health team. Must be able to work 1 weekend a month.

About the Role:
Join LMVNA&rsquo;s compassionate team as a Registered Nurse (RN) delivering skilled nursing care in patients&rsquo; homes. You&rsquo;ll care for individuals with a wide range of medical needs &mdash; from routine to complex &mdash; and make a meaningful impact by helping them reach their highest level of independence.
What You&rsquo;ll Do:<ul><li>
Conduct comprehensive nursing assessments (including OASIS) at SOC, ROC, discharge, and other milestones</li><li>
Develop and maintain individualized, patient-centered care plans</li><li>
Administer medication management and reconciliation</li><li>
Educate patients and caregivers to support independence</li><li>
Obtain timely physician orders and ensure proper documentation</li><li>
Case manage patients, coordinating care across interdisciplinary teams</li><li>
Communicate effectively with physicians, supervisors, and care teams</li><li>
Supervise LVNs and Home Health Aides (CHHAs)</li><li>
Ensure regulatory compliance with Medicare notices (HHABN, NOMNC)</li><li>
Participate in quality improvement and training initiatives</li><li>
Accurately report hours, mileage, and maintain documentation standards</li><li>
Serve as a role model and preceptor as needed</li></ul>
Requirements:

 What We&rsquo;re Looking For:<ul><li>
Licensed Registered Nurse (RN) in the state of California</li><li>
Strong clinical judgment and critical thinking skills</li><li>
Current BLS certification.</li><li>
Home Health or acute care experience preferred</li><li>
OASIS experience highly desirable</li><li>
Excellent communication and organizational skills</li><li>
Ability to work independently and manage a flexible schedule</li><li>
Commitment to delivering high-quality, compassionate care</li><li>
One year professional work experience as a Registered Nurse.</li><li>
Bilingual in Spanish preferred</li></ul>
