<div>Long Island Care Center is seeking a qualified Nurse Practitioner to join their clinical team in a skilled nursing and post-acute care setting. 
Bilingual- Chinese (Mandarin and/or Cantonese speaker preferred) 
Description: 
The NP will provide comprehensive primary care, manage chronic conditions, and perform acute assessments to minimize hospital readmissions.
Key Responsibilities:<ul>
<li>Conduct comprehensive physical exams and assessments of sub-acute and long-term care residents.</li>
<li>Collaborate with nursing staff, physicians, and multidisciplinary teams to develop and implement care plans.</li>
<li>Manage acute and chronic illnesses, including ordering diagnostic tests, prescribing medications, and ordering treatments.</li>
<li>Provide timely documentation within the EMR system.</li></ul>
Qualifications:<ul>
<li>Active, unencumbered Nurse Practitioner license (FNP, AGNP, or ANP).</li>
<li>Prior experience in skilled nursing, post-acute care, or geriatric care preferred.</li>
<li>Strong communication and collaboration skills.</li></ul>
