Your energetic approach and critical thinking will drive exceptional patient care and foster trusting relationships with clients and their families.Duties<ul><li>Deliver comprehensive nursing assessments, including vital signs, physical examinations, and patient history evaluations to determine individualized care plans.</li><li>Administer medications, injections, IV infusions, and treatments such as tube feeding or catheter care while adhering to all safety protocols and HIPAA regulations.</li><li>Monitor patients&rsquo; health status using advanced EMR (Electronic Medical Records) systems like Epic or Cerner to document medical records accurately and efficiently.</li><li>Manage complex cases involving dementia care, Alzheimer's care, memory care, and patients with developmental disabilities or behavioral health needs.</li><li>Provide specialized interventions such as wound care, IV insertion, sterile processing techniques, and acute pain management for diverse patient populations.</li><li>Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning, post-acute care, or hospice services.</li><li>Educate patients and families on health coaching topics including medication administration, nutrition, infection control, and disease management strategies.</li></ul>Qualifications<ul><li>Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).</li><li>Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable.</li><li>Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks for accurate documentation.</li><li>Experience working with individuals with disabilities or chronic illnesses including Alzheimer&rsquo;s disease and dementia is preferred.</li><li>Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.</li><li>Ability to perform physical examinations, venipuncture (blood draws), phlebotomy, and sterile procedures confidently.</li><li>Excellent patient assessment skills combined with compassionate communication to support diverse populations including pediatrics and geriatrics.
