Experienced Consultant Pharmacist &ndash; Skilled Nursing Facility (SNF)-Central FloridaWe are seeking a highly skilled and detail-oriented Consultant Pharmacist to provide clinical and regulatory oversight for our Skilled Nursing Facility (SNF). The Consultant Pharmacist is responsible for ensuring safe, effective, and compliant medication use for all residents, in accordance with federal and state regulations, including CMS guidelines.This role involves performing monthly medication regimen reviews (MRRs), identifying and resolving medication-related issues, and providing clear, actionable recommendations to the interdisciplinary team. The Consultant Pharmacist plays a key role in supporting quality assurance and performance improvement (QAPI) initiatives, reducing survey risk, and enhancing resident outcomes.The ideal candidate will have strong clinical knowledge in geriatrics, excellent communication skills, and a deep understanding of long-term care regulations, including psychotropic medication management, gradual dose reductions (GDRs), and documentation standards. This position may also include staff education, policy development, and participation in facility committees.Key Responsibilities:<ul><li>Conduct monthly medication regimen reviews and document findings per regulatory requirements</li><li>Identify adverse drug events, unnecessary medications, and opportunities for optimization</li><li>Provide recommendations to physicians and nursing staff</li><li>Ensure compliance with CMS, state, and facility policies</li><li>Support QAPI initiatives and survey readiness</li><li>Educate staff on medication safety and best practices</li></ul>Qualifications:<ul><li>Active Consultant Pharmacist license for Florida</li><li>Experience in long-term care or consultant pharmacy</li><li>Strong knowledge of CMS regulations and SNF standards</li><li>Excellent clinical judgment and communication skills</li></ul>Pay: $56.82 - $68.43 per hourWork Location: Remote
