Job Title:</b> Registered Nurse Case Manager</p><p><b>
Job Overview:</b></p><p>
As RN Case Manager of Luminos Hospice, you will work with the Clinical Director to lead a care team including LPN(s), LVN(s), Aides, MSW and Chaplain to provide best-in-class hospice care.</p><p><b>
Responsibilities</b></p><ul><li>
Coordinates and oversees all aspects of patients' home care.</li><li>
Collaborates with patient care teams to develop and implement care plans.</li><li>
Educates families on patient condition, needs and care.</li><li>
Performs head to toe physical assessments.</li><li>
Checks vital signs and manages medications.</li><li>
Administers and maintains catheters.</li><li>
Complete documentation timely and accurately.</li><li>
Supervises HHAs.</li><li>
Regularly communicates patient progress to the clinical manager, physician and care team.</li><li>
Other duties as assigned.</li></ul>
Requirements:
<ul><li>RN license in the state of Texas</li><li>
Current driver&rsquo;s license, reliable transportation, valid auto insurance</li><li>
Current CPR and BCLS certification</li><li>
Excellent verbal and written communication skills</li><li>
Two (2) years' experience as a registered nurse in a clinical care setting, hospice preferred</li></ul><p><b>

We&rsquo;re seeking a motivated and relationship-driven Hospice Marketing Liaison to grow awareness of our services and connect more patients and families with quality care. In this role, you&rsquo;ll build strong community partnerships, support our team, and help expand Luminos Hospice&rsquo;s mission across the region.</p><h2 class="jobSectionHeader"><b>
What You&rsquo;ll Do</b></h2><ul><li>Grow and manage a portfolio of accounts between College Station and Brenham, including assisted living, memory care, skilled nursing, hospitals, and physician offices.</li><li>Build and maintain strong referral relationships with healthcare providers and community partners.</li><li>Participate in staff engagement efforts and serve as a bridge between our team and community partners.</li><li>Conduct presentations, outreach visits, and community events to educate others on hospice services.</li><li>Track referral data and collaborate with the care team to ensure smooth patient transitions.</li></ul><h2 class="jobSectionHeader"><b>

Requirements:
<ul><li>2+ years in healthcare marketing, business development, or liaison role (hospice or home health preferred).</li><li>Excellent communication and relationship-building skills.</li><li>Self-motivated, goal-oriented, and able to work independently.</li><li>Valid driver&rsquo;s license and reliable transportation.</li></ul></div>
