Your Role:<ul><li>
Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered.</li><li>
Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents.</li><li>
Create a service plan in conjunction with the client to develop goals and objectives that are consistent with what the client wishes to accomplish.</li><li>
Counsel, advocate, and teach clients the skills identified in their service plan face to face in home or virtually.</li><li>
Respond to client&rsquo;s crisis situations within program parameters in a timely manner according to program protocols.</li></ul>
What We&rsquo;re Looking For:<ul><li>
Bachelor's or Master's Degree in Social Work, Counseling, Psychology, Criminal Justice, or other Human Behavior Science.</li><li>
Candidate must be a licensed driver with access to dependable personal transportation and auto insurance</li><li>
Passion for making a difference and supporting families through life&rsquo;s challenges.</li><li>
Preferred 2 years experience with Child Welfare, Adoption, Foster Care, Community Resources or Care Coordinator.</li></ul>
