Position Summary:</b></h3><p>
The Director of Admissions and Case Management is responsible for coordinating the resident admission process while providing ongoing case management services that promote residents&rsquo; social, emotional, and mental well-being. This position serves as a key liaison between residents, families, referral sources, and internal departments to ensure a seamless transition into care and continued high-quality, person-centered support throughout each resident&rsquo;s stay.</p><p></p><h3 class="jobSectionHeader"><b><br>
Qualifications:</b></h3><ul><li>
Bachelor&rsquo;s degree in Human Services, Social Work, Healthcare Administration, or related field required</li><li>
Minimum of two years&rsquo; experience in admissions, case management, or healthcare marketing required</li><li>
Strong interpersonal, communication, and organizational skills</li><li>
Ability to manage multiple priorities and exercise sound judgment</li><li>
Experience working with older adults and families in healthcare or residential settings</li><li>
Knowledge of community resources, social service programs, and assisted living regulatory requirements</li></ul><h3 class="jobSectionHeader"><b>
Salary:</b></h3><ul><li>
Starting at $60,405.80, commensurate with experience</li></ul><h3 class="jobSectionHeader"><b>
Essential Job Responsibilities:</b></h3><ul><li><b>
Manage the full admissions process</b> from application to move-in, ensuring timely communication, accurate documentation, and regulatory compliance.</li><li><b>
Conduct pre-admission assessments</b> to determine eligibility, appropriate placement, and readiness for transition.</li><li><b>
Serve as primary liaison</b> with hospitals, referral partners, families, and community agencies to facilitate smooth referrals and admissions.</li><li><b>
Collaborate with Marketing and Administration</b> to support outreach, census growth, and relationship-building initiatives that strengthen community awareness.</li><li><b>
Coordinate interdisciplinary care planning</b> by partnering with nursing, therapy, dietary, and finance departments to promote continuity and quality of care.</li><li><b>
Provide case management services</b>, including social assessments, counseling, and assistance with accessing benefits and community resources.</li><li><b>
Monitor residents&rsquo; well-being</b> and coordinate support for social, emotional, and behavioral health needs, including crisis intervention and counseling referrals.</li><li><b>
Facilitate discharge and aftercare planning</b> to ensure smooth transitions and continuity of support post-discharge.</li><li><b>
Maintain accurate records and reports</b> for admissions and case management, including resident case notes, census trends, and regulatory documentation.</li><li><b>
Represent Hultquist Place professionally</b> in community events and referral networks, promoting the facility&rsquo;s mission, values, and reputation for excellence in resident care.</li></ul><h3 class="jobSectionHeader"><b>
