<div><b>About the Role</b></div>
<div>As a Behavioral Health Clinician, you&rsquo;ll be doing the important, meaningful work of managing crises, providing in-the-moment phone support to clients with complex behavioral health issues, ensuring client safety, and connecting clients to high-quality, evidence-based providers and facilities. This position does not involve providing in-person services or therapy but instead focuses on supporting clients in connecting to care, exclusively by phone and email.</div>

<div>This role's internal title is: Clinical Care Navigator. The position can be carried out fully remote/virtually. Remote candidates must be physically located within the United States.</div>
<h3 class="jobSectionHeader"><b>Responsibilities</b></h3><ul>
<li>Using evidence-based interventions, skillfully manage client crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting the client to treatment</li>
<li>Identify roadblocks to clinical care options for our clients using Lyra&rsquo;s proprietary software and other methods to connect clients to the ideal treatment, based on specific needs and preferences</li>
<li>Independently manage a caseload of complex, multi-need clients</li>
</ul><h3 class="jobSectionHeader"><b>Qualifications</b></h3><ul>
<li><b>Master&rsquo;s degree in clinical social work or equivalent</b>; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc) or a Bachelor's degree in nursing (RN). We&rsquo;re unable to provide supervised hours toward licensure. Candidates without a state license to deliver mental health services, independently, will not be considered (with the exception of the RN license)</li>
<li>Must have 2+ years psychiatric intake, assessment RB, other related experience</li>
<li>EAP/clinical call center experience preferred</li>
<li>Must be Bilingual, Spanish-speaking</li>
<li>Significant experience managing high-risk clinical situations (e.g., suicidality, mandated reporting)</li>
<li>Must be a full time resident of the United States; work cannot be completed from outside the United States.</li>
</ul><div><b>Pay and Benefits</b></div>
<div>As a full-time Clinical Care Navigator, you will be employed by Lyra Health, Inc. The anticipated base rate for this full-time position is $41 per hour. The base rate is determined by role and level, and the base rate will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience and location.</div>

 As a contract Child and Adolescent nurse practitioner, you will provide essential clinical care. This role emphasizes core competencies such as diagnostic expertise, family engagement, ethical practice, and continuous improvement to ensure the highest standard of care.

   Practice comprehensive mental health medication management via a 60-minute intake appointment and regular follow-up appointments
  </li>
  <li>
   Conduct mental health risk assessments, psychiatric medication evaluations, and longitudinal medication treatment
  </li>
  <li>
   Construct patient-centered treatment plans, document treatment recommendations, message patients, and prescribe and manage psychiatric medications
  </li>
  <li>
   Experience and commitment to providing safe, evidence-based medication treatment that follows clinical best practice guidelines (e.g., measurement based care, patient-centered care, paying close attention, medication minimizing risks, and maximizing benefits)

   Graduate from an accredited (CCNE) Psychiatric-Mental Health Nurse Practitioner (PMHNP) program; OR graduate from an accredited PA program (ARC-PA) with certification through PANCE with preference for Certificate of Added Qualifications in psychiatry, post-graduate fellowship in psychiatry, or other academic psychiatry certificate program completion, including a Doctor of Medical Science with Psychiatry concentration
  </li>
  <li>
   Unrestricted state license in Oregon
  </li>
  <li>
   A minimum of 2 years in practice
  </li>
  <li>
   Strong preference for NPs and PA-Cs with an active DEA license
  </li>
  <li>
   Preference for NPs and PA-Cs with additional certification or clinical experience with minors
  </li>
  <li>
   Must have office space with ability to see clients
  </li>
  <li>
   Ability to provide care to patients with a full range of diagnoses, clinical complexity, and severity levels, with a preference for providers who offer in-person options
  </li>
