JOB SUMMARY
The duties of a Palliative Care, Post-Acute Nurse Practitioner, APRN mid-level provider include, but are not limited to, making rounds in the skilled nursing facilities, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients. MUST HAVE/QUALITY OF LIFE/ PALLIATIVE CARE EXPERIENCE.
Monday-Friday and alternating Saturdays

We focus on early needs identification, collaborative care planning, and aligning treatment with each person&rsquo;s goals.

 Cognitive Assessments 
We include formal cognitive screenings at intake to identify early decline, support communication, and ensure appropriate surrogate decision-making where needed.
&#65039; Goals of Care Conversations 
We guide patients and families through structured conversations about what matters most&mdash;making sure care aligns with values, preferences, and long-term priorities.
 Discharge &amp; Functional Planning 
We support transitions from hospitals or SNFs by helping families understand a patient&rsquo;s new level of function and prepare for home care, outpatient follow-up, or other support services.

Our team works closely with PCPs, specialists, and discharge planners to ensure care is person-centered, coordinated, and values-driven. We meet people where they are and help them move forward with clarity and dignity. 
PRIMARY FUNCTIONS<ul><li>
Examine patients and record history</li><li>
Adhere to proper charting protocols</li><li>
Discuss with Physician and other health care professionals to prepare comprehensive plan</li><li>
Order or execute tests or images as needed</li><li>
Submit health care plan and goals of individual patients for periodic review by Physician</li><li>
Analyze results of tests, diagnose and report to patient</li><li>
Administer or prescribe treatment and medications</li><li>
Promote health by advising patient on diet, hygiene and prevention of disease</li><li>
Inoculate and vaccinate as required</li><li>
Refer to specialist as required for patient's well-being</li><li>
Report deaths and contagious diseases to governmental authorities</li><li>
Other responsibilities as assigned</li></ul>
EDUCATION AND EXPERIENCE<ul><li>
Must have and maintain current State of Florida license in good standing</li><li>
Current National Board Certification</li><li>
Current DEA</li><li>
Post Acute experience preferred</li></ul>
KNOWLEDGE, SKILLS, AND ABILITIES<ul><li>
Effective written, verbal and electronic communication skills</li><li>
Show initiative, creativity and problem-solving ability</li><li>
Ability to collaborate with colleagues to provide quality healthcare</li><li>
Experience in Palliative care</li></ul>
 ADDITIONAL QUALIFICATIONS<ul><li>
Prior experience using an electronic medical records system</li><li>
Bilingual preferred (English/Spanish)</li></ul>
RELATIONSHIP REPORTING
Reports to the Supervisory Palliative Physician
PHYSICAL REQUIREMENTS<ul><li>
Ability to communicate effectively with patients</li><li>
Ability to collaborate with colleagues as necessary to provide quality health care</li><li>
Ability to use a computer, printer and scanner</li><li>
Ability to sit, stand or view a computer screen for extended periods of time</li><li>
