Position Overview:
The Physical Therapist Assistant (PTA) supports the delivery of high-quality rehabilitative care under the supervision of a licensed Physical Therapist. This role focuses on implementing individualized treatment plans that improve residents&rsquo; mobility, reduce pain, enhance strength, and restore physical function. The PTA collaborates closely with the interdisciplinary care team to ensure consistent, resident-centered therapy. A strong foundation in geriatric rehabilitation, clinical precision, and a compassionate approach are essential to support optimal outcomes and promote residents&rsquo; independence and quality of life 
Key Responsibilities:
Clinical Support and Treatment Implementation<ul><li>
Deliver physical therapy interventions as outlined in the treatment plan developed by the supervising Physical Therapist.</li><li>
Assist residents with therapeutic exercises, gait training, and functional mobility activities to improve strength, balance, and independence.</li><li>
Monitor resident responses during treatment sessions and communicate progress, concerns, or changes to the Physical Therapist.</li><li>
Support resident and family education under the direction of the Physical Therapist to
promote safe and effective carryover of therapy strategies.</li><li>
Maintain a safe, supportive, and engaging therapeutic environment that encourages resident participation and comfort.</li><li>
Participate in interdisciplinary team meetings and contribute to care planning discussions to ensure coordinated, resident-centered care.</li></ul>
Documentation and Compliance<ul><li>
Accurately document treatment sessions, resident performance, and observations in
accordance with facility policies and regulatory standards.</li><li>
Adhere to HIPAA, OSHA, and infection control protocols to ensure compliance and resident safety.</li></ul>
Collaboration and Operational Support<ul><li>
Maintain open communication with the supervising Physical Therapist, interdisciplinary care team, and resident care staff to support continuity of care.</li><li>
Assist in maintaining therapy equipment and supplies, reporting any issues promptly to
ensure a safe and efficient therapy environment</li></ul> 
Qualifications:<ul><li>
Associate&rsquo;s degree from an accredited Physical Therapist Assistant program</li><li>
Current state licensure as a Physical Therapist Assistant</li><li>
1&ndash;2 years of experience in a skilled nursing facility or geriatric care setting preferred</li><li>
Familiarity with PDPM and SNF therapy documentation standards is a plus</li></ul>

Position Overview:
The QMA is a member of the community nursing team whose responsibility is set up, administer, and record certain prescribed medications for residents in the community under supervision of a licensed nurse and as allowed by state and federal regulatory agencies.

Administrations must be administered safely and documented.</li><li>Prepare and administer topical medications to intact skin.</li><li>Administer PRN drugs, as allowed, when authorized by a license nurse.</li><li>Check physician orders against medication forms.</li><li>Maintain cleanliness and proper storage of all drugs while maintaining a clean working environment.</li><li>Keep medication cart locked while unattended.</li><li>Care for and position all resident tubing and bags as instructed.</li><li>Make rounds and report needs and conditions to the charge nurse as applicable.</li><li>Administer vital signs and document properly.</li><li>Keep medical records current by charting pertinent resident conditions timely and routine charting as scheduled, with co-signature by a licensed nurse.</li><li>Order medications from pharmacy per physician order.</li><li>Use appropriate drug reference if ever unsure or unclear as to drug being administered.</li><li>Count all drugs on Individual Control Drug Records with on-coming charge CTM at shift change.</li><li>Answer call lights, bed/chair sensors, and security care bracelet (if applicable) alarms promptly and courteously.</li></ul>
Qualifications:<ul><li>Certified as a Qualified Medication Aide in the state employed; certificate must be active, valid, and in good standing.</li><li>Certified as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing.</li></ul>
