Position Overview:
The Social Service Director direct and manages the day-to-day operations of the Social Service department in accordance with current federal and state regulations in addition to the community policy.
Key Responsibilities:<ul><li>Supervise the social services department in the day-to-day operations ensuring to set job assignments and set priorities.</li><li>Develop goals and objectives for the department; motivate and manage care team members to accomplish the same.</li><li>Ensure new care team members are effectively oriented; ensure an ongoing educational program is in place.</li><li>Develop positive relationships on behalf of the community with regulatory agencies, residents, and families including participating during family orientation and family council.</li><li>Assist in preparing budget for the department; administer approved budget.</li><li>Implement policies/procedures with follow-up and supervision of care team members to ensure compliance.</li><li>Provide direct psychosocial intervention.</li><li>Perform resident assessments at admission, upon condition change, and/or annually.</li><li>Create, review, and update care plan and progress notes.</li><li>Provide direct psychosocial intervention.</li><li>Coordinate resident visits with outside services, dental, optical, etc.</li><li>Attend and document resident council meetings.</li><li>Assist resident families in coping with skilled nursing placement, physical illness, and disabilities of the resident as well as the grieving process.</li><li>Work with the resident, family, and other care team members to plan discharge.</li><li>Conduct in-service programs to educate staff regarding psychosocial issues and patient rights.</li></ul>
Qualifications:<ul><li>Bachelor's degree in social work or related field.</li><li>Certified as required by the state of employment.</li><li>One year of social work experience in a long-term care environment.</li><li>One-year supervisory experience in a setting service setting.</li></ul>
