Position Summary: The Licensed Practical Nurse (LPN) assists and supports the Resident Services Director with the overall implementation, delivery, and coordination of nursing services in the community.

Key Responsibilities:

<li>Medication administration</li>
<li>Direct care and service to residents</li>
<li>Teaching and wellness counseling</li>
<li>Health Promotion</li>
<li>Participation in assessments under the supervision of the Resident Services Director (RN)</li>
<li>Support of residents and families</li>
<li>Oversight of Certified Nursing Assistants</li>
<li>Training of associates and auditing of care</li>
<li>Completing monthly in-service training as required</li>

Education/Experience/Licensure/Certification
<ul>
<li>Licensed Practical Nurse currently licensed to practice in the CT state</li>
<li>Two years full time or full time equivalent clinical experience</li>
<li>Experience and/or special interest working with older adults</li>
<li>Prior dementia and psych experience strongly preferred but not mandatory</li>
<li>Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 50lbs.</li>
<li>Ability to maintain positive working relationships with residents, their families, peers and other staff members</li>
<li>In good physical and emotional health and free of communicable diseases</li>

Key Responsibilities:
<ul>
<li>Provide outstanding customer service to all residents, family members, responsible persons, and visitors to the residence</li>
<li>Coordinate and deliver nursing care to residents within the scope of an LPN, including but not limited to catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and management of medical equipment; medication management including administration of oral medications, injections or PRN medications.</li>
<li>Supervise the care provided by HHAs and RCAs to residents.</li>
<li>Manage and administer medications and assist the residents with self-administration of medication.</li>
<li>Maintain positive collaborative relations with residents, families and provider partners</li>
<li>Report any observed significant incidents and/or changes in the resident&rsquo;s condition to Nursing Leadership (Charge RN in the Residence), Primary Care Provider and family as indicated.</li>
<li>Review and become familiar with each assigned resident&rsquo;s Individualized Service Plan.</li>
<li>Assist in training and orientation of Resident Care staff.</li>
<li>Document care and services provided to residents in the electronic health record using nursing best practices and documentation standards.</li>
<li>Follow all Insp&#299;r policies and procedures as outlined in the policy and procedure manual</li>
<li>Attend and participate in scheduled in-service programs, training programs and staff meetings.</li>
<li>Assist with scheduling and staffing of Resident Care staff as needed.</li>
<li>Provide coaching and mentoring to Resident Care Team members as needed.</li>
<li>Follow emergency procedures.</li>
<li>Understand and comply with all Insp&#299;r safety practices and procedures.</li>
<li>Participate in health, wellness and lifestyle programs and assist with all aspects of resident care and resident experience as needed</li>
<li> May perform other duties as assigned.</li>
</ul>
Education/Experience/Licensure/Certification:
<ul>
<li>Must be a Licensed Practical Nurse, licensed to practice in the state of New York with one of the following:</li>
<li>Associates degree and a minimum of two (2) years of full time, or the equivalent of full time, clinical experience in nursing, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of the sick at home</li>
<li>A diploma or associates degree in nursing and at least four (4) years of full-time, or the equivalent of full-time, clinical experience in nursing within the past ten (10) years, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of the sick at home</li>
<li>Basic first aid and CPR certification.</li>
<li>Experience in assisted living, post-acute or long-term care setting preferred.</li>
<li>Experience with and/or special interest in working with seniors.</li>

 The Human Resources Generalist plays a critical role in leading the day-to-day operations of the human resources function within the community with the goal of reducing turnover and improving associate satisfaction. This role is responsible for overseeing HR activities, ensuring alignment with company policies and fostering a positive work environment. Key responsibilities include recruitment and onboarding, employee relations, training and development, progressive discipline, payroll coordination and benefits administration, and community culture.

 Validate job requisitions against budgetary constraints and organizational needs to ensure accurate and necessary hiring.

 Create compelling and accurate job postings across various platforms, including company career sites, job boards, and social media.

 <li>
 Review resumes and applications to identify qualified candidates, conduct initial phone screens, and assess fit for the role and company culture.
 </li>
 <li>
 Schedule and conduct interviews with candidates, collaborating with hiring managers to evaluate skills, experience, and cultural fit. Provide feedback and recommendations to decision-makers.

 Manage the offer process by preparing offer letters, negotiating terms with candidates, and ensuring timely communication.

 Collaborate with Executive Director to complete employee investigations including but not limited to: employee/resident complaints, policy and procedure violations, and other employment matters.

 Advise and assist community leadership in executing progressive discipline processes.

 Performance Management:
 </li>
 <ul>
 <li>
 Support the performance evaluation and development process, ensuring timely and constructive feedback is provided to employees.
 </li>
 <li>
 Work with managers to address performance issues and develop improvement plans through expanded use of talent management (i.e., Red/Yellow/Green process).
 </li>
 <li>
 Facilitate the exit interview process and analyze turnover trends to improve retention strategies.
 </li>
 <li>
 Perform and track 30, 60, 90-day check-ins on new hires and conduct stay interviews with current employees to lengthen tenure and strengthen community culture.
 </li>
 </ul>
 <li>
 Payroll Coordination and Benefits Administration:
 </li>
 <ul>
 <li>
 Responsible for accurate data entry into the HRIS including but not limited to pay changes, status changes, classification changes etc.
 </li>
 <li>
 Ensures complete and accurate timecard entries and schedule usage as well as training Department Heads on time and attendance system.
 </li>
 <li>
 Serving as the first point of contact for payroll and benefit inquiries for community employees.
 </li>
 <li>
 Assist with the new hire enrollment and administration of employee benefits and compensation programs.
 </li>
 <li>
 Support benefits communications and open enrollment at the community level.
 </li>
 <li>
 Collaborate with accounting on any payroll issues in a timely manner.
 </li>
 <li>
 Manages disbursement of payroll checks and encourages employees to enroll in direct deposit.
 </li>
 </ul>
 <li>
 Community Culture:
 </li>

 Advance a positive and inclusive community culture by promoting employee engagement and satisfaction.

 Attend regular staff meetings to stay informed and address any HR-related concerns.

 Ensure the confidentiality of sensitive employee information and maintain compliance with privacy regulations.

 Administer all people processes including leave management, Workers Compensation, OSHA, EEO reporting and unemployment claims with coordination from Home Office HR team.
