Certified Nursing Assistants at Masonicare at Mystic assist residents with activities of daily living and personal care to maximize independence, freedom of choice and quality of life. The official title for this role is &ldquo;Residential Care Assistant&rdquo;.</p><p><br>
 CNA - Essential Duties and Responsibilities:<br>
<br>
 1. Provides assistance with essential activities of daily living and personal care to residents based on age, needs and abilities of the resident, including bathing, dressing and undressing, ambulating, toileting, assistance with meals and nourishment's, personal hygiene (shaving, dental hygiene, nail, hair and foot care), transferring using proper body mechanics and appropriate assistive devices and care of ADL devices such as eyeglasses and hearing aids.<br>
<br>
 2. Provides assistance with instrumental activities of daily living to residents based on age, needs and abilities of the resident: placing and/or receiving telephone calls, performing light housekeeping tasks such as vacuuming, emptying trash, cleaning bathroom, bed making and bed changes, assisting with laundry, escorting the resident and coordinating internal transportation needs and assisting with mail delivery and correspondence<br>

 3. Assists the resident with self-administration of medications, through cueing only</p><p><br>
 4. Observes and reports changes in the residents' physical condition and cognitive/emotional status to ALSA Supervisor or Director of Assisted Living Services, as needed</p><p>
5. Conducts room checks on resident rounds, monitors for environmental safety hazards, records incidents, errors or accidents and records resident and resident related events.<br>

 5. Assists in a variety of tasks related to dining (escorting residents, set-up and clean-up, serving meals)<br>
<br>
 6. Responsible for participating in and supporting the resident centered activity program. Completes required written documentation concerning resident care and activities during shift</p><p>
7. Is a resident advocate at all times</p><p>
8. Follows agency policies concerning confidentiality</p><p>
9. Uses tactful, diplomatic communication in potentially sensitive or emotionally charged situations and follows up with appropriate staff, resident or others regarding reported<br>
 complaints, problems and concerns.<br>
 10. Acts as ambassador to guest and other off-campus visitors and maintains certifications required for employment<br>

Responsible for carrying out the plan of care for the adult, geriatric, pediatric, and neonatal client as developed by the nurse case manager while addressing all problems in the plan of care, identifying interdisciplinary care needs and making recommendations to the nurse case manager for additional home health care services.</li></ul></div><div></div><div><br>
<ul><li>
Assists nurse case manager by providing coordination of the plan of care, maintaining continuity of client care with other health professionals assigned to the case, communicating changes in status to other professionals/paraprofessionals and participating in client care conferences</li></ul></div><div></div><div><br>
<ul><li>
Supervises patient care rendered by Licensed Practical Nurses. Supervises, educates and conducts supervisory visits for home health aides.</li></ul></div><div></div><div><br>
<ul><li>
Provides education, supervision and counseling to the adult, geriatric, pediatric, and neonatal client and family regarding home nursing procedures and other care needs as appropriate to client&rsquo;s needs. Utilizes agency educational material as appropriate.</li></ul></div><div></div><div><br>
<ul><li>
Documents timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.</li></ul></div><div></div><div><br>
<ul><li>
Participates in the agency&rsquo;s continuous quality improvement program.</li></ul></div><div></div><div><br>
<ul><li>
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.</li></ul></div><div></div><div><br>
<ul><li>
Can be depended upon to report to work on-time, use time off appropriately, and complete annual education &amp; medical requirements.</li></ul></div><div></div><div><br>

**This role will work on a 50 bed Behavioral Health Hospital Unit**</p><p>
The <b>Behavioral Health Nurse</b> provides professional nursing care to emotionally disturbed and/or mentally ill patients for Behavioral Health. Works with clinical and case management staff to coordinate care and assist in treatment plans for patients; and to perform related work. Practices Evidence-Based nursing.</p><p></p><p><br>
Essential Duties and Responsibilities:</p><ul><li>
Assumes the professional responsibility and accountability for a group of residents/patients including:<ul><li>
Medication/Treatment Administration</li><li>
Teaching</li><li>
Supervision of all nursing staff to assure implementation of the interdisciplinary care plan.</li></ul></li><li>
Conducts intake assessments through interviews with patients and relatives to gather case history. Performs ongoing and systematic assessments utilizing the nursing process to establish a comprehensive goal directed interdisciplinary plan.</li></ul><ul><li>
Plans and provides nursing care for patients.</li><li>
Communicates and collaborates effectively with residents/patients, families, colleagues and other members of the interdisciplinary team to assure that treatment is appropriate to meet the age, needs and abilities of resident/patient and unit needs.</li></ul><ul><li>
Administers and notes reactions to psychotropic drugs and other medications. Arranges further medical attention when necessary.</li><li>
Assists the client in understanding the nature of emotional disturbances and accepting the need for treatment.</li><li>
Assists in maintaining a safe and secure environment for staff and patients.</li><li>
Tests for vital signs of pain and ensures treatments of pain.</li><li>
Documents all patient information on medical charts and follows physician&rsquo;s written orders.</li><li>
Maintains patient confidentiality.</li><li>
Maintains accurate records and documentation of patient treatments, immunizations and laboratory results according to standard medical recordkeeping.</li><li>
Performs administrative assignments such as filing, answering phones, typing, and completing progress notes in the computer system.</li><li>
Provides courteous, high quality service to patients and members of the public by personally responding to requests for service or making appropriate referral.</li><li>
Supervises the care provided by CNAs &amp; LPNs assuring adherence to the plan of care, resident profile, policies and procedures.</li><li>
Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory programs.</li><li>
Participates in quality improvement activities as directed.</li><li>
Follows standards of the Diligent Minimal Lift Program.</li><li>

Minimum Qualifications:</b></p><p>
Education: Graduate of accredited school of nursing. BSN Preferred.</p><p>
Experience: Minimum one year clinical experience, preferably in an acute and/or Behavioral Health capacity.</p><p>

Current RN license in the State of Connecticut, in good standing<br>
