JOB PURPOSE/SUMMARY

Summary of position/role of team:
<ul><li>The Nurse Manager will provide administrative and clinical oversight of assigned staff in their cohort and their respective programming.
</li><li>Responsible for the supervision, training, and oversite of all nursing staff across their assigned cohort.
</li><li>Oversees scheduling and program needs within their designated crisis cohort.
</li><li>Other duties include onboarding, training, and development of newly hired RNs, assisting the Nursing Supervisor with ensuring the delivery and oversight of psychiatric care to patients involved in medication services provided at the Center's crisis programming.
</li><li>Work closely with a team of nurses, providers and Directors to address other related clinical and administrative needs.
</li><li>Oversees the quality of nursing services and ensures appropriate care is being provided.
</li><li>Provide direct clinical services under HRMC protocols. An essential job function is a reliable and on site, with regular attendance or whatever the job requires.</li></ul>

TYPICAL WORKING CONDITIONS/ENVIRONMENT
<ul><li>
The Nurse Manager will work throughout the Center in their assigned crisis cohort including on an as needed basis, but will not be working out in the community.
</li><li>The Nurse Manager is expected to cover any open shifts in either crisis cohort if unable to obtain coverage.</li></ul>

1. Supervise and manage members of assigned nursing team.
<ul><li>
Supervises one of the two assigned cohorts: EmPATH, C-CSU, BHUCC, CP, H-CSU, &amp; K-CSU.</li><li>
Participates in the recruitment of nursing staff.</li><li>
Participates and establishes training expectations for nursing staff.</li><li>
Provides supervision for assigned RNs &amp; LPNs in their cohort</li><li>
Provide regular feedback to assigned RNs &amp; LPNs regarding their proficiency and performance.</li><li>
Supervise and manage nursing coverage in their assigned cohort and assists with other crisis programming.</li><li>
Rotate on-call schedule and cover open shifts if unable to obtain coverage</li><li>
Manage timesheets and leave request for assigned staff members</li><li>
Conduct employee evaluations for assigned staff</li></ul>

2. Lead by example.
<ul><li>
Address disciplinary issues, to include progressive disciplinary action if needed in a timely fashion.</li><li>
Display superior emotional adjustment and stability with a non- reactive and non-judgmental attitude to crisis.</li><li>
Display ability to implement change in a reluctant environment as needed.</li><li>
Demonstrate effective delegation as needed.</li><li>
Foster relationships with external stakeholders including nursing agencies and local universities.</li></ul>

3. Works in a multi-disciplinary team approach to meet the clinical needs of patients.
<ul><li>
Represents the Center in a professional manner.</li><li>
Attend and participate in meetings and on committees as designated.</li><li>
Collaborate with management and other team members around patient care to deliver high quality services.</li><li>
Assist with monitoring productivity requirements.</li></ul>

4. Completes all documentation in compliance with CARF and HRMC standards
<ul><li>
Completes and signs all documentation for each client prior to end of shift</li><li>
Respond to all flags, emails, and voicemails from other clinical staff by end of shift. The RN is to return calls within 15 minutes</li><li>
Document administered medications in the EMAR Document clinical services provided in the medical record on same day as service provided in a clear and concise manner.</li></ul>

5. Completion of required HRMC training to maintain status as credentialed provider
<ul><li>
Maintain an active RN license in the state of Tennessee in accordance with the state guidelines</li><li>
Complete any Center Required Self-Directed Learning or Training Modules in the required time frames.</li><li>
Complete First Aid, CPR, and HWC trainings within the required time frames.</li></ul>

Schedule:
<ul><li>The Nurse Manager is expected to fulfill the agreed upon hours when hired.
</li><li>Unless otherwise approved, the Nurse Manager is expected to be present in the clinic from 8am-5pm and is expected to complete a minimum of 16 hours of clinical work per week which may include nights and/or weekends.
</li><li>Must be flexible with scheduling as the Nurse Manager will rotate being on call and is expected to cover open shifts in their assigned cohort on a PRN basis.
</li><li>The Nurse Manager may also be asked to cover open shifts in the other cohort on a PRN basis.
</li><li>All other scheduling arrangements are approved on a case-by-case basis.</li></ul>

Travel:
<ul><li>Must possess a valid Tennessee Driver's License.
</li><li>This position requires utilizing a personal vehicle to conduct Center business.
</li><li>Maintaining a dependable vehicle and certified driver status is a condition of employment.
</li><li>The Nurse Manager may have to travel between clinics/programs at times, and may be required to travel for any supervision or scheduled meetings.
</li><li>There is no overnight travel and clients will not be transported.</li></ul>

Equipment/Technical Competency:
<ul><li>Basic computer skills required (EMR, Excel, Word, etc.).
</li><li>Must be familiar with EMRs and telehealth services as telehealth services may be offered as part of the service delivery.</li></ul>

Equipment/Technology:
<ul><li>The Nurse Manager will receive a center issued laptop.
</li><li>The Nurse Manager will need access to e-mail, Athena Health (EMR), Drs. First, access to I &amp; U Drives, and VPN (remote) access.</li></ul>

QUALIFICATIONS - RN Manager

Experience / Knowledge:
<ul><li>1-3 years of psychiatric nursing post-graduation is required.
</li><li>Prior supervisory experience is preferred.
</li><li>Should possess sound knowledge of state regulations and best practice guidelines, psychiatric medications, mental health diagnoses, treatment modalities and interventions.
</li><li>Must have the ability to present professionally, work within a team format, exercise sound clinical judgment under pressure, communicate effectively (both orally and written), possess good time management and organizational skills.
</li><li>Experience with interdisciplinary collaboration and care coordination preferred.
</li><li>Knowledge of HIPAA, confidentiality, and patient centered practice approach required.</li></ul>

Education / License:
<ul><li>Must have graduated from an accredited school of nursing and possess a Bachelor's degree in nursing.
</li><li>Must be actively licensed as a Registered Nurse in the state of Tennessee.
</li><li>American Nursing Association certification as a Psychiatric Mental Health Nurse preferred.</li></ul>

Physical/Emotional/Social - Skills/Abilities:
<ul><li>
Staff must complete and maintain First-Aid and CPR trainings within the required time frames.
</li><li>The Nurse Manager must complete the full Handle With Care Trainings as indicated.
</li><li>Some exposure to biological hazards and unpleasant odors.
</li><li>Hearing of normal and soft tones.
</li><li>Close eye work.
</li><li>Lifting up to 50 lbs.
</li><li>Pushing/pulling up to 150 lbs.
</li><li>Frequent sitting, standing, walking, bending, stooping, and reaching.</li><li>
This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.</li></ul>

The FEI Crisis Team Leader

Duties:
<ul><li>Responsibilities include managing two teams of four staff total, serving a minimum of 15 families at time for a period of 90 days post-crisis intervention, per team.</li><li>
Duties also include meeting with families, coordinating supports for monthly Family Team Meetings (FTM), and organizing monthly family engagement activities.</li><li>
Essential duties include providing program oversight and coordination of referrals and caseloads, including but not limited to linkage and referral to area resources, advocacy and coordination of care with other provider and social supports, crisis intervention and planning.</li></ul>

JOB PURPOSE/SUMMARY

Summary of role of team:
<ul><li>&quot;Family Empowerment Initiative&quot; or &quot;FEI&quot; provides support to low-income families, children ages birth (0) to eighteen (18), and their caregivers.</li><li>
This includes, but is not limited to, intensive services, crisis services, self-sufficiency through trauma informed services, case coordination, peer support, parent/caregiver education, and family engagement strategies.</li></ul>

Summary of position:
<ul><li>The Team Leader provides administrative and clinical supervision for two FEI teams, and performs all administrative duties required to adhere to the guidelines of the program.
</li><li>The Team Leader provides information and referral, agency linkage, advocacy, home visits, and follow-up services as defined by the FEI program and ensures that each team member on the team does the same for consumers on their caseloads.</li><li>
The Team Leader arranges coverage plans for their team to ensure that each consumer is seen for required program contacts per month.</li><li>
The Team leader provides crisis intervention and emergency services as needed.</li><li>
The Team Leader utilizes center wide and community services as needed for client benefit.</li><li>
The Team Leader embraces the key values of care coordination: empowerment, normalization, rehabilitation, and continuity of care.</li><li>
Ongoing networking with crisis providers for timely access to appropriate referrals and overall community outreach are required.</li></ul>

TYPICAL WORKING CONDITIONS/ENVIORNMENT
<ul><li>
This position spends the majority of their time working at Knox Children &amp; Youth (C&amp;Y) Center and requires both office-based clinical and administrative work and community-based clinical interactions.</li><li>
Team Leader will be responsible for engaging clients, peers, and community stakeholders in a professional and therapeutic manner.</li></ul>

<ul><li>
Attends and is timely to weekly multi-disciplinary Team Mtgs.
</li><li>Attends and leads administrative team meeting on a weekly basis without tardiness.</li><li>
Returns all staff calls within 30 minutes as reported by supervisor.</li><li>
Responds to all flags, emails and voicemails within 2 business days.</li><li>
Responsible for ensuring all phases and activities of the Wraparound Process are done to the high quality and fidelity - accountable to family team.</li><li>
Attends required trainings by TDMHSAS or associated partners to meet program requirements</li></ul>

2. Completes all required documentation in compliance with Center Policy, CARF and SSOC standards.
<ul><li>
Ensures tx plan is complete within 30 days of admission with input from other professionals as evidenced by signatures of APN/MD and Peer Support Worker</li><li>
Completes Crisis Plan, DLA-20, Individual Service Plan (ISP) and eligibility assessment on all clients</li><li>
Completes No Harm contract at ct.'s first sign or report of SI, HI, or thoughts of harmful behavior.</li><li>
Progress notes within one working week of encounter.</li><li>
Ensures that all clients have a current ROI for their PCP and other active providers (daycare, school, therapists, additional providers, etc&hellip;)</li><li>
Completes seven-day contact on all post-hospital clients.</li><li>
Complete an assessment of the family including Child and Adolescent Needs and Strengths (CANS) assessment, Caregiver Strain Assessment, and other national evaluation tools to develop a care plan.</li></ul>

3. Engages clients and their families in the treatment process providing treatment planning, linkage, referral, and advocacy for each client.
<ul><li>
Ensures each client is contacted within 24 hours of referral</li><li>
Creates goals with the ct. based on their input as well as input from family/other systems AEB treatment plan signatures by all parties.</li><li>
Creates and implements treatment interventions based on the treatment plan goals.</li><li>
Utilizes family strengths and empowers the family to implement effective interventions as evinced by documentation of activity in the progress notes.</li><li>
Makes referral to less intensive services upon anticipation of discharge</li><li>
CM works with the ct. for up to 90 days to ensure transition to less intensive services.</li><li>
The Team Leader monitors outcome measures to ensure quality and effectiveness (QA/QI) and quarterly QRR.</li></ul>

4. Ensures that the program meets monthly requirements
<ul><li>
Continually monitors admissions and referrals to ensure timely assignment to caseloads
</li><li>Monitors productivity on a weekly basis in supervision AEB document tracking logs</li><li>
Ensures coverage is met for clients when staff are out on leave or during vacancies
</li><li>Ensures vacancies are filled with qualified candidates AEB use of applicant screening profiles</li><li>
Submits monthly report to supervisor by the 5th business day of each month.</li></ul>

5. Provides administrative supervision to FEI teams
<ul><li>
Provides support to staff through positive reinforcement, open communication, honest feedback and constructive criticism on a daily basis.</li><li>
Completes written documentation of verbal and written corrections pertaining to staff and forwards to HR.</li><li>
Meets with each case manager individually on a weekly basis for supervision</li><li>
Shares information re: policies and procedures with staff as policies change</li><li>
Ensures each staff is following Center Policy and Procedure via monitoring in weekly sessions and periodic chart reviews.</li><li>
Assists staff with difficult cases by attending home visits with the staff on an as needed basis.</li><li>
Makes suggestions for interventions with clients on a weekly basis</li><li>
Provide continuous check-ins with team members.</li></ul>

COMPENSATION:
<ul><li>Starting salary for this position is approximately $21.08/hr based on relevant experience and education.</li></ul>

Schedule:
<ul><li>Primary working hours are Monday through Friday, and should be provided at times that meet the needs to the families served.</li><li>
On site supervision is preferred, and position must be available via phone if a function of the position requires being out of office.</li></ul>

Travel:
<ul><li>This position requires utilizing a personal dependable vehicle to conduct Center business.</li><li>
Maintaining a dependable vehicle and certified driver status is a condition of employment.</li></ul>

Equipment/Technical Competency:
<ul><li>Ability to use a computer and basic office software is required, including but not limited to Office Word, Excel, Outlook and be able to utilize EMR for clinical documentation.</li></ul>

Equipment/Technology:
<ul><li>This position requires the use of computer skills with an electronic medical record (EMR), and telehealth/televideo services may be offered as part of the service delivery.</li><li>
Staff will be expected to maintain work with a Center cell phone and computer.</li></ul>

QUALIFICATIONS - FEI Crisis Team Leader

Experience / Knowledge:
<ul><li>Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual/developmental disabilities.
</li><li>Two (2) years work experience with SPMI populations preferred.</li><li>
Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.</li><li>
Clinical staff members must possess reliable transportation, a valid driver's license with an F endorsement OR the ability to acquire an F endorsement on driver's license in order to transport clients as needed.
</li><li>Transporting clients in a personal or Center vehicle is an essential function of this position.</li></ul>

Education / License:
<ul><li>A Bachelor's degree in a health related field of counseling, psychology, or social work or must be licensed as a nurse (LPN or RN).
</li><li>LPNs must have fifteen (15) semester hours of behavioral health and two (2) years of behavioral health care experience.
</li><li>Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of coursework in behavioral health and at least two (2) years of work experience in the behavioral health setting working with children.
</li></ul>Physical/Emotional/Social - Skills/Abilities:
<ul><li>
Outpatient/Community based environment.
</li><li>This position requires training and certification in and the ability to adequately implement a variety of verbal de-escalation methods and personal/client safety techniques.
</li><li>Must have mental ability to exercise sound judgment under pressure.
</li><li>Must have an F endorsement and a valid Driver's License.
</li><li>This position requires utilizing a personal dependable vehicle to conduct Center business.
