If you're committed to excellence and compassionate patient care, we encourage you to apply.POSITION TITLE &amp; OVERVIEW: The Provider (Nurse Practitioner or Physician Assistant) will educate the patients on how to create a lifestyle of wellness that will ensure they have a long and productive life! This is a foundational approach to preventing and treating many chronic diseases. Truly help change lives!The Provider will work under the supervision of the medical director. Under general direction, will provide patient care; conduct physical exams; complete health history summary; document patients' progress and assists with patient care management.RESPONSIBILITIES:<ul><li>Obtain and review patient medical history information relevant to patient care</li><li>Conduct physical examinations</li><li>Order and review laboratory studies as well as diagnostic procedures</li><li>Assume responsibility for prescribing FDA approved appetite suppressants according to state and federal guidelines</li><li>Review patient progress and manage patient care accordingly</li><li>Maintain all required documentation for patient care and medication management</li><li>Conduct patient consultations</li><li>Provide training to clinical and non-clinical staff as needed</li><li>Document patient progress notes in the patient chart and/or EMR.</li><li>Conduct consultations with patients regarding the program</li><li>Perform other clinical duties as required</li></ul>QUALIFIED APPLICANTS MUST HAVE:<ul><li>Must have valid Medical &amp; DEA License.</li><li>Must ensure CAQH profile is current.
