This Community Care Coordinator communicates consistently and effectively with various partners and community providers and other Metrocare staff regarding clients&rsquo; assessments, barriers, and unique needs. This may include housing, benefits, food and clothing resources, and other areas to ensure a complete and comprehensive treatment approach. The Community Care Coordinator will provide connection to services for individuals coming through various community partners that seek to reduce the impact of mental health and social determinants that lead to crisis. They will be expected to work closely and collaboratively with community mental health agencies, law enforcement, and service providers regarding clients to develop effective responses to referrals. The team will also provide updates and feedback on referrals that are made and will communicate and coordinate with community providers regarding services for referrals. 
 
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
<ul><li>Provide coordination in all aspects of care between the clients, community partners and Metrocare.
</li><li>Assist clients while they are in crisis in accessing appropriate mental health services and facilitate appropriate continuity of care.
</li><li>Provide accurate and timely reporting metrics to supervisors.
</li><li>Make accurate and effective assessments and recommendations regarding the client&rsquo;s mental health needs in compliance with the client&rsquo;s rights as well as all relevant State and Federal statutes, and Metrocare policies and procedures.
</li><li>Develop and maintain rapport with the various agencies and organizations involved with the diversion program.
</li><li>Prepare, create, and maintain records, files, documents, reports, and correspondence both within Metrocare and partner agencies and organizations.
</li><li>Work with other Metrocare employees, members of partner agencies and organizations, as well as the target population and their families in a constructive and efficient fashion.
</li><li>Create and/or enhance an existing person/family-centered recovery plan that identifies an individual&rsquo;s goals, objectives, strengths and preferred involvement of natural supports as well as coordination with internal and external providers.
</li><li>Assessing consumer needs on a continual basis throughout the course of treatment
</li><li>Assistance with scheduling and completing all internal and external provider appointments.
</li><li>Assistance to meet needs impacting social drivers of health including housing, food, clothing, transportation, employment and academic needs.
</li><li>Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services.
</li><li>Coordination with internal and external providers to ensure a holistic approach to care that considers all areas of an individual&rsquo;s life, the individual&rsquo;s wants and goals and all treatment interventions.
</li><li>Documenting services in the electronic health record within 24 hours after services have occurred.
</li><li>Develop/maintain collaborative working relationships with internal and external referral resources, allowing creation/renewal of informal and formal agreements between Metrocare and those external agencies. Relationships with external agencies will include but are not limited to: FQHCs, inpatient psychiatric and substance use facilities, the Department of Veteran Affairs, inpatient acute care hospitals and hospital outpatient clinics and community/regional supports and providers such as schools, child welfare agencies, criminal justice, and Indian Health Services.
</li><li>Assists the individual in service in developing and/or strengthening natural support who will participate in care coordination activities.
</li><li>Maintains confidentiality of information concerning consumers and family members
</li><li>Duties and projects may be assigned or changed to meet business needs. Hours and shift assignments may be outside of normal business hours.

QUALIFICATIONS
 
EDUCATION AND EXPERIENCE:
<ul><li>Required: Bachelor&rsquo;s Degree in social work, psychology, criminal justice, or related human services field
</li><li>Minimum of 2 year of experience in behavioral health care, working with individuals with severe persistent mental illness and/or substance use disorders
</li><li>Strong knowledge base of Dallas County community providers and resources to include behavioral and physical health care, housing, transportation, employment/academic supports, and other social service agencies.
</li><li>OR
</li><li>Master&rsquo;s degree in psychology, Social Work, or related field.
</li>Preferred: Bilingual Spanish, experience with communicating with third parties and building rapport with community partners. Knowledge of Federal and State benefits available to client. LMSW or equivalent

Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The QMHP-Adult Intensive position provides psychosocial/rehabilitative service to adults with serious mental illness. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, engagement into treatment, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. The overall goal of this position is to maximize the individuals potential level of functioning, reduce hospitalization and aid in the successful reintegration of individuals into the community. 
 
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:

</li><li>Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to individuals with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.
</li><li>Ensure authorization for clinical services.
</li><li>Formulate individual recovery plan based on assessment findings on all new admissions.
</li><li>Develop measurable objectives and goals agreed upon by the individual.
</li><li>Actively assist in obtaining and modifying goals as needed.
</li><li>Document all attempts to involve individual, service providers, and caregivers (unless opposed by the individual) in service plan process.
</li><li>Review service plans every 180 days or as clinically indicated.
</li><li>Participate in interdisciplinary team staffing. Present psychosocial assessment findings. Update team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might affect course of treatment.
</li><li>Follow-up with assigned individuals who miss a scheduled clinic appointment. Document attempts to contact individual. Report repeated unsuccessful efforts to contact the treatment team.
</li><li>Oversees and assists assigned individuals medication adherence. Provides med training, arranges transportation to scheduled office based appointments, and prompts needed refills.
</li><li>Document clinical services by close of next business day.
</li><li>Perform follow up hospital assessments the same day as requested by the hospital and report assessment outcome to the Clinical Manager
</li><li>Provides crisis intervention/on-call services during the work week and by weekend rotation. Make home and hospital visits, initiates mental illness warrants, work with police and other public servants as a needed to address crisis for assigned clients
</li><li>Facilitate inpatient admission upon request of the treatment team and/or hospital. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning with hospital. Meet with the individual within 2 days of discharge in the community.
</li><li>Evaluate progress of clinical session, solicit feedback from individual(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
</li><li>Co-facilitate family education workshops.
</li><li>Identify and assist individuals in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
</li><li>Participation in a required on call rotation.
</li>Performs other duties as assigned.

EDUCATION AND EXPERIENCE:
<ul><li>Required: Bachelor's Degree from an accredited college/university with a minimum of 30 credit hours in a social, behavioral, or human services field. The credit hours include but are not limited to the following course types: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, special education, early childhood/early childhood intervention, physician assistant, gerontology, and educational psychology.Required: 1-3 years of Mental Health Related Experience
