About Us
 We are looking for a motivated and experienced Service Coordinator to join our team at Mortimer House. Mortimer House is a busy and fast-paced service supporting up to 21 people with learning disabilities, dementia, sensory impairments, and complex health needs. We have a large, dedicated staff team who are well regarded by families and professionals, and who are committed to delivering high-quality, person-centred care. This is a key role within the service, supporting the day-to-day running of the home and helping ensure safe, effective, and responsive care for the people we support.

The Role
 <ul>
<li>Supporting the day-to-day management of the home</li>
<li>Leading and coordinating shifts, providing direction and support to staff</li>
<li>Coordinating people’s care and leading a Planning Team</li>
<li>Writing, reviewing, and updating care plans and risk assessments</li>
<li>Supporting with rota management, staffing allocation, and shift coordination</li>
<li>Leading handovers and supporting team communication</li>
<li>Supporting safe medication practice and care delivery</li>
<li>Monitoring standards within the service and supporting good practice</li>
<li>Liaising with families, professionals, and external agencies</li>
<li>Supporting audits, compliance, and service improvements</li>
<li>Working closely with the Service Manager and Nursing Team to maintain a high-quality service</li>
</ul>
About You
 <ul>
<li>Experience working within a care setting, ideally in a senior or supervisory role</li>
<li>Confidence leading shifts and supporting a staff team</li>
<li>Strong understanding of person-centred care</li>
<li>Organised and able to manage competing priorities</li>
<li>Confident using IT systems (care records, emails, rotas)</li>
<li>Strong communication skills with staff, families, and professionals</li>
<li>Motivated to improve outcomes for the people we support</li>
<li>Ability to manage concerns or issues within the service</li>
</ul>

Charnhill Crescent is a small service providing support to five mature adults, giving the people we support every chance to live their best life! Whether that’s through gaining more independence, learning a new skill, we work with every individual to make sure that they can live their lives as they choose.
 This is an interesting and varied role, which would suit a confident, self-motivated , creative person who enjoys meeting people and can use their initiative to identify and capitalise on opportunities for development.
 We are a small team who support people who live in St George, in a communal home. However, this role may require working across two other services in Mangotsfield and Staple Hill.
 
Shifts start at 9am and consist of long days and shorter mid-shifts and include sleep-ins, but may vary and are flexible around what the individuals would like support with. Support hours include some evenings and weekends and lone working will also be a requirement of the role, so being self-motivated and able to use your initiative is crucial.
 
As Support Worker, your responsibilities will include: 
<ul>
<li>Supporting people to manage a tenancy</li>
<li>Develop strategies and resilience to manage their own mental health</li>
<li>Developing independent living skills</li>
<li>Budgeting</li>
<li>Support to manage relationships and much, much more.</li>
</ul>
The successful Support Worker will: 
<ul>
<li>Be person-centred in your approach, encouraging, inspiring and can think creatively</li>
<li>Have excellent active listening and communication skills and can adapt your skills and knowledge to the needs of the people you will be supporting</li>
<li>Are flexible, proactive, non judgemental and passionate about supporting people to achieve their goals</li>
<li>Hold a current driving license and can use your own car for work would be an advantage.</li>
