Job DescriptionMinnesota Hospice is a locally owned and operated community hospice in the Dakota, Scott and Rice County areas. Our leadership team is dedicated to the success of all through a variety of different activities that include but are not limited to, educational opportunities, trainings, and team building activities.Job Summary:This position is responsible for providing personal care related to the daily living activities of hospice patients. The RN Case Manager (RNCM) is a key component and is responsible for facilitating key relationships within the communities and facilities they serve.Supervisory Responsibilities: &middot; Certified Nursing Assistant (CNA)&middot; Licensed Practical Nurse (LPN)Duties/Responsibilities:&middot; Assume primary responsibility for the patient and family needs that includes assessing, planning, implementing and evaluating care needs.&middot; Maintain communication with providers and staff members to coordinate optimal care and use of resources for the patient, family and facility.&middot; Provide education regarding medications, side effects, dosing instructions, etc. to the patient, family and facility.&middot; Assess psychological, social and spiritual needs of the patient and family and notify the appropriate social worker and/or chaplain when warranted to meet the needs of the patient.&middot; Coordinate and communicate medication changes and orders with the Nurse Practitioner and or hospice physician.&middot; Coordinate delivery of medication, equipment and supplies with patients, family and facilities.&middot; Communicate equipment and supply needs with the Director of Nursing.&middot; Assist in determining the need to adjust the level of care provided based on nursing assessment in collaboration with the Director of Nursing, Hospice Physician, Nurse Practitioner, and attending physician.&middot; Document all on-call activity including but not limited to, phone calls and visits in the EHR within 24 hours.&middot; Document all routine and PRN visits, phone calls, and patient interactions in the EHR within 72 hours.&middot; Evaluate and update patient care plans and/or care plan problems every 14 days.&middot; Provide appropriate guidance at the time of death including but not limited to, witness medication disposal, notification to county coroner, funeral home and postmortem care.&middot; Implement care to achieve outcomes of a comfortable and safe death.&middot; Conduct monthly supervisory visits with LPN and Hospice Aides.&middot; Maintain communication with facilities and caregivers through the hospice binder, calendar and visit record note entries each visit.&middot; Abe to demonstrate the competency to complete the initial comprehensive assessment upon hospice admission.&middot; Ability to assist with and operate patients with physical and mechanical transfers.&middot; Ability to properly stage a wound and obtain treatment plan from provider. Communicating the same plan to family, caregiver and facilities as appropriate.Required Skills/Abilities:&middot; Knowledge of appropriate HIPAA and confidentiality&middot; Maintain accurate documentation&middot; Able to work as both a part of a team and independently&middot; Possesses great organizational Skills&middot; Excellent time management skills.&middot; Excellent written and verbal communication skills&middot; Valid driver&rsquo;s license and auto insurance&middot; Ability to use EHR systems and operate mobile devices with efficiency&middot; Knowledge of Microsoft products i.e. Excel, Word, etc.Education and Experience: &middot; Graduate of Accredited Nursing Program: minimum associate degree (AND); bachelor&rsquo;s degree (BSN) preferred&middot; Posses a valid Minnesota Registered Nurse (RN) License&middot; Current CPR/BLS Certification&middot; Minimum of 1 year nursing experience in healthcare setting&middot; One year hospice or long term care experience preferred&middot; Willingness to obtain the certification in Hospice and Palliative Care within 2 years of employment.Physical Requirements: &middot; Able to regularly lift and move up to 50lbs&middot; Regularly operate and assist with physical and mechanical transfers&middot; May have prolonged periods of sitting at a desk and working on a computer&middot; Personal protective equipment provided, including masksCompany Expectations: &middot; Adhere to all Minnesota Hospice policies and procedures&middot; Promote teamwork&middot; Lead by example&middot; Conduct yourself in a positive, confident and professional manner&middot; Communicate clearly, concisely and accurately&middot; Promote the hospice concept within the communities we serve&middot; Offer a willingness to participate in Minnesota Hospice team building and community events&middot; Posses a valid driver&rsquo;s license and auto insurance coverageThis job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employer.
