 Hospice Registered Nurse (RN)

Shifts Available: Full Time

Service Area for Coverage: Northwest &amp; Southwest Houston

Responsibilities:
<ul>
<li>Oversee nursing care team.</li>
<li>Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.</li>
<li>Plan, organize, and direct home care services.</li>
<li>Conduct assessments of patient care needs and develop care plans.</li>
<li>Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.</li>
<li>Prepare timely, complete, and accurate clinical notes.</li>
<li>Night and Weekends are required per hospice needs of patients</li>
</ul>
Qualifications:
<ul>
<li>Valid RN license in the State of Texas.</li>
<li>Hospice experience (at least 1 year).</li>
<li>Knowledge of home health regulations and general healthcare compliance.</li>
<li>Experience with Home Care Home Base (HCHB) EMR (preferred but not required).</li>
<li>Excellent communication and people skills are essential.</li>
<li>Ability to work independently and as part of a team.</li>

Responsibilities
<ul>
<li>Participate in the QOL, QAPI meetings conducted in the community.</li>
<li>Follow systems, nursing standards and guidelines.</li>
<li>Coordinate and oversee the Medicare Advantage procedures for the facility.</li>
<li>Timely and accurate completion of residents&rsquo; care plans/assessments/PASRR and take follow-up action as necessary.</li>
<li>Participate in the pre-admission process to determine potential residents&rsquo; level of care and participate in resident level of care determination upon admission.</li>
<li>Support the billing schedules and procedures.</li>
<li>Be familiar with residents&rsquo; condition and care needs and follow and educate community associates on the review process and provide recommendations to address potential areas of concern/opportunity, utilizing programs and reports on various state and company generated reports.</li>
<li>Provide education to community associates on POC documentation, change of condition alerts; late and mid-loss ADL&rsquo;s to support MDS and QM data elements.</li>
<li>Provides information to regional operations, clinical and community management team on the status of clinical and financial reimbursement.</li>
<li>Participates in all training as required including CEU 360, PCC, SimpleLTC/TMHP, Nethealth and company program rollouts. Associate Commitment &ndash; Clinical Care Coordinator 04/01/2019 Page 2</li>
<li>Participate in clinical assignments for comprehensive reviews as part of community IDT.</li>
<li>Conduct audits of each communities EHR/documentation systems.</li>
<li>Perform other duties as assigned by the community administrator.</li>
</ul>
Qualifications:
<ul>
<li>Current/valid Texas RN or LVN nursing license or a current/valid RN/LVN/LPN license from a Compact Party State. (The RN/LVN may continue to practice under the former home state license and multistate licensure privilege for a period not to exceed thirty days.)</li>
<li>Preferred Skilled Nursing Facility Experience a Management level nurse Traits/Skills/Other Requirements</li>
<li>Proficient in use of computer systems (Word, Excel, Electronic Health Record system (PCC), etc.)</li>
<li>Must have excellent command of English language.</li>
<li>Must possess the ability to make decisions using discretion and independent judgment with respect to matters of significance to the Company.</li>
<li>Must be in good standing with the Federal and State Government</li>
