<div><div><p>The <b>Moments Hospice Triage RN</b> is a key member of the interdisciplinary team, providing after-hours clinical support to patients and families. In this fully remote role, you will assess needs, provide guidance, and coordinate care using the nursing process and Telecare protocols.</p>

This position is ideal for an experienced hospice RN who is confident working independently while delivering compassionate, patient-centered care.</p>
<p><b>
What You&rsquo;ll Do</b></p>
<ul>
<li>Provide after-hours triage, clinical assessment, and interventions via phone</li>
<li>Support patients and families with symptom management and end-of-life care needs</li>
<li>Coordinate care by collaborating with physicians, facilities, and internal teams</li>
<li>Dispatch visits and coordinate services with vendors as needed</li>
<li>Deliver clear, compassionate education and support to families</li>
<li>Accurately document all interactions, care plans, and outcomes in the EMR</li>
<li>Review team documentation for accuracy and compliance</li>
<li>Partner with Telecare staff to ensure positive patient outcomes</li>

Hours for this position will be Monday-Friday 12:00 pm-8:00 pm</b></p>

<h3 class="jobSectionHeader"><b>Responsibilities:
</b></h3><div><ul><li>Participation in the information gathering, analysis, education and form completion aspects of the admissions process</li>
<li>Evaluate patients for hospice appropriateness, e.g. medical records review as requested.</li>
<li>Obtain agreement for admission of referred patients and families who meet admission criteria.</li>
<li>Analyze data collected to facilitate/obtain admission orders</li>
<li>Build effective referral source relationships to support the high quality and responsiveness expectations associated with Moments Hospice</li>
<li>Maintain clinical and professional competence.</li>
<li>Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.</li>
<li>Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient&rsquo;s family has concerning hospice and palliative care</li>
</ul></div><h3 class="jobSectionHeader"><b>Requirements:</b></h3><div>
<ul><li>Valid Registered Nurse (RN) license</li>
<li>Driver's license, insurance and vehicle</li>
<li>1-2 years of nursing experience (preferred)</li>
<li>Utmost compassion and integrity</li>
<li>Currently licensed as a R.N. in the state where duties are performed.</li>
<li>Previous hospital, physician practice, LTC or ALF experience</li>
<li>Strong Presentation Skills</li>
<li>Previous sales or service experience preferred</li>
