<div>JOB SUMMARY:
As aHospice Aideyou will perform personal care activities related to the activities of daily living, under direction of a Case Manager RN.
QUALIFICATIONS:<ul><li>
Must have reliable vehicle.</li><li>
12 months of caregiving experience required (home care, assisted living/memory care, skilled nursing/nursing home, hospital or hospice),</li><li>
Hospice experience preferred</li><li>
Excellent customer service skills</li><li>
Current CPR Certified</li><li>
Valid driver's license and auto insurance</li></ul>
RESPONSIBILITIES (including but not limited to):<ul><li>
Personal cares (bathing, dressing, grooming, toileting, skin care, etc)</li><li>
Review patient care plan</li><li>
Lifting and transferring</li><li>
Assistance with range of motionpositioning, exerciseambulation</li><li>
Assistance with diningnutrition</li><li>
Electronic charting</li></ul>
In addition:<ul><li>
Needs strong understanding of professional boundaries</li><li>
Must be able to lift 50 lbs</li><li>
Good understanding of body mechanics</li><li>
Strong communication skills-written, verbal, listening with attention</li><li>
Good problem solving skills</li><li>
Good time management of self and others</li><li>
Able to function in an electronic medical record</li></ul>
