Key Responsibilities</b></h2><ul><li>Resident Care &amp; Support</li><li>Provide compassionate, hands-on care to residents, including assistance with bathing, grooming, dressing, toileting, and mobility.</li><li>Assist residents with meals, snacks, hydration, and feeding as needed.</li><li>Support residents with ambulation, positioning, and range-of-motion activities.</li><li>Monitor residents&rsquo; comfort levels and promptly address needs and concerns.</li><li>Health Monitoring &amp; Documentation</li><li>Monitor and record vital signs, weight, and fluid intake/output accurately.</li><li>Report any changes in residents&rsquo; physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.</li><li>Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.</li><li>Resident Engagement &amp; Activities</li></ul><p></p><h2 class="jobSectionHeader"><b>

Key Responsibilities</b></h2><ul><li>Medication Management &amp; Documentation</li><li>Prepare, administer, and document medications in accordance with Mosaic policies, physician orders, and state regulations.</li><li>Maintain accurate and timely entries in the Medication Assistance Record (MAR).</li><li>Verify, clarify, and update physician orders as needed.</li><li>Order, receive, and securely store medications in compliance with community protocols.</li><li>Maintain a clean, organized, and secure medication room.</li><li>Resident Care &amp; Support</li><li>Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and ambulation.</li><li>Take and record vital signs, monitor health changes, and communicate updates promptly to the Clinical Manager or Executive Director.</li><li>Provide first aid and wound care when necessary and coordinate care with outside healthcare professionals as directed.</li></ul><p></p><h2 class="jobSectionHeader"><b>
