 MyCare Health Center is seeking a Medical Assistant to join our multi-location health center in Macomb County! The position of Medical Assistant performs assistance to medical staff in the examination and treatment of patients under general supervision. All medical assistants actively participate in the Patient Care Team by pre-planning office visits, morning group huddles, educating patients and orienting them to improve their health.

 State of Michigan Certification or ability to obtain it within 90 days is required. The ideal candidates will have 1+ years of experience in medical assisting in a Family Practice setting. Must have flexibility M-F 8:00 AM to 7:00 PM. This position may be assigned to either our Center Line or Mt Clemens location, with flexibility to work at either site as requested.  SPECIFIC DUTIES:

 <ul>
  <li>
   Assists medical staff in the examination and treatment of patients, e.g. office visits, procedures.
  </li>
  <li>
   Assists with direct patient care procedures and related tasks; checks in patients, assists in obtaining patient histories, takes vital signs, prepares charts, and assists with medical examinations; all part of the medical home experience at MyCare.
  </li>
  <li>
   Prepares exam rooms for examination of patients and maintains stocks of medical supplies as necessary in the exam rooms.
  </li>
  <li>
   As ordered by the Provider, administers injections.
  </li>
  <li>
   As ordered by the Provider, performs venipuncture blood draws and processes specimens according to required lab specifications.
  </li>
  <li>
   As ordered by the Provider, performs CLIA waived testing according to CLIA and manufacture&rsquo;s guidelines.
  </li>
  <li>
   Educates and advises patients on specified medical issues within established parameters, including: medication refills, laboratory, diagnostic results, appointments and hospital admissions.
  </li>
  <li>
   Under the direction of a Physician, Nurse Practitioner or Physician Assistant, assists in all aspects of medication prescribing, e.g. prior authorizations, calling in refills, medication reconciliation.
  </li>

   Documents services delivered in a timely, accurate and prescribed manner, in compliance with the MyCare Health Center Quality Assurance Plan
  </li>
  <li>
   Adheres to the Policies and Procedures, standards for service delivery and Code of Ethics established by MyCare Health Center
  </li>
  <li>
   Practices all applicable safety, environmental, and infection control methods.
  </li>
  <li>
   Informs and assists Provider/Supervisor of any patient that needs immediate medical attention.
  </li>
  <li>
   Handles all phone calls in a timely and courteous manner.
  </li>
  <li>
   Assist the supervisor as unusual problems arise.
  </li>
  <li>
   Support staff in assigned project based work.
  </li>
  <li>
   Other duties as assigned by immediate supervisor, including filling in at the front desk as needed

 MINIMUM QUALIFICATIONS:

   Completion of a technical education program for medical assisting and Medical Assistant Certification is required within 90 days of employment, if not currently certified.
  </li>

   1-2 years of medical assisting experience in a family medicine environment is preferred.
  </li>
  <li>
   All employees are required to maintain a current BLS/CPR certification and training will be provided if needed.

Medical Assistant (Full-time)

<div></div><div><div><b>GENERAL DESCRIPTION:</b></div>
<div></div><div>The C2C LMSW Case Manager is a split position with .5 FTE dedicated to providing comprehensive case management and care coordination services to individuals with chronic or complex physical and/or behavioral health needs, with a focus on those recently released from incarceration. The TCM works as part of a multi-disciplinary care team to ensure patients gain access to appropriate medical, behavioral health, substance use disorder (SUD), educational, and social services. Services include assessments, planning, linkage, advocacy, monitoring, education, and follow-up support. The remaining .5 FTE for this position will be focused on providing therapeutic services to adult patients diagnosed with Behavioral Health Disorders and Substance Use Disorders (as well as other co-occurring disorders). Responsibilities include, but are not exclusive to: psychosocial assessments, diagnoses, individual therapy, group therapy, treatment planning, psychoeducation, referrals and consultations.</div>
<div></div><div><b>SPECIFIC DUTIES AND JOB FUNCTIONS</b></div>
<div></div><div>1. Conduct comprehensive assessments of medical, behavioral, and social needs, particularly for individuals transitioning from incarceration.</div>
<div>2. Develop, implement, and monitor individualized care plans in collaboration with the patient, care team, and other providers.</div>
<div>3. Facilitate linkage to primary care, behavioral health, SUD services, and community resources.</div>
<div>4. Provide education, counseling, and ongoing support to patients and their families regarding health conditions, treatment plans, and recovery goals.</div>
<div>5. Coordinate care transitions and advocate on behalf of patients to reduce barriers to accessing care.</div>
<div>6. Participate in an integrated team approach to patient care/treatment. Treatment team will include physicians, RNs, peer recovery coaches, clinicians, etc.</div>
<div>7. Monitor patient outcomes, reassess needs, and adjust care plans as appropriate.</div>
<div>8. Maintain timely and accurate documentation in accordance with Medicaid, state, and federal requirements.</div>
<div>9. Collaborate with Medicaid health plans, corrections systems, and community partners to ensure continuity of care for justice-involved individuals.</div>
<div>10. Respond to crises and provide appropriate interventions within scope of practice.</div>
<div>11. Provide individualized behavioral health treatment to consumers: including initial psychosocial assessment, diagnoses, individualized treatment plans and on-going therapy sessions</div>
<div>12. Maintain a therapy caseload that appropriately meets the needs of patients, utilizing evidence-based practices, best care practices, industry standards, organizational guidelines, appropriate accreditation standards, and applicable federal laws, including HIPAA</div>
<div>13. Refer patients for more specialized or inpatient interventions as clinically appropriate.</div>
<div>14. Maintain quality care through participation in peer review, weekly case review meetings, policy development and quality improvement programs as requested</div>
<div>15. Complete all documentation in an appropriate, timely manner and in accordance with third party compliance standards</div>
<div>16. Maintain and update professional knowledge and proficiency through continuing education, staff meetings and other resources specific to providing integrated behavioral and mental services in a federally qualified health center</div>
