</div><div><b>Schedule: Sunday - thursday 7:00Am - 3:30PM</b></div>
<div></div><div><div><b>POSITION SUMMARY</b><b>:</b></div>
<div>Under the direct supervision of the Clinical Supervisor, the Case Manager ensures coordination and continuity of care for all clients, provides skill training and development, completes intake assessment, facilitates groups, connects clients to community resources, and provides coordination of care.</div>
<div></div><div><b>RESPONSIBILITIES</b><b>:</b></div>
<ul><li>Completes initial intake/assessments for new clients and completes all related paperwork in a timely manner.</li>
</ul><ul><li>Assists with or facilitates health promotion, psychoeducational and skills training groups; provides individualized case management to Outpatient (Integrated Health) clients.</li>
<li>Updates Treatment plans for clients, as needed.</li>
<li>Completes clinical documentation on daily basis including but not limited to group and individual progress notes, intake and discharge documentation.</li>
<li>Assists with development of individual and client cultural competencies.</li>
<li>Provides support around addressing Social Determinants of Health needs including documentation of:
<ul><li>Workforce development, helping with resumes, interviews as indicated by client need.</li>
<li>Referrals for all necessary basic needs, supplies, i.e., medical appointment, eyeglasses, and other aids.</li>
<li>Referral for services for housing, employment, job training, etc.</li>
<li>Assist with transportation resources as needed.</li>
<li>Assist in obtaining benefits such as with applications for food stamps, GA, AHCCCS.</li>
</ul></li><li>Assists in making community referrals and support.</li>
<li>Assists clients in completing the housing interest form and VI-SPDAT</li>
<li>Assists with coordination of care amongst treatment team and referral sources and multidisciplinary team, including Adult Recovery Team meetings.</li>
</ul><ul><li>Provides coverage for Front Desk as needed.</li>
<li>Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.</li>
<li>Completes duties using trauma informed care principles.</li>
</ul><ul><li>Other duties as assigned.</li>

<div></div><div><b>EDUCATIONAL/WORK EXPERIENCE REQUIREMENT</b><b>:</b></div>
<ul><li>Associates of Arts Degree in behavioral health field and two (2) years of experience in substance use or mental health treatment, or</li>
<li>High school diploma or GED and 4 years&rsquo; experience</li>
<li>Strong knowledge of community resources and referral sources</li>
<li>Possess and maintain a valid Arizona driver&rsquo;s license and</li>
<li>Possess or able to obtain a Fingerprint Clearance Card</li>
<li>CPR/First Aid Certificate within 90 days of hire</li>
</ul>
<div></div><div><b>SKILLS REQUIREMENT</b><b>:</b></div>
<ul><li>Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors</li>

<li>Understand the dynamics of chemical dependency and demonstrate the ability to integrate the knowledge for practical application</li>
<li>Must understand the recovery process of the chemically dependent individual
</li><li>Experience in working with the Native American population preferred</li>
<li>Know the economic, educational and social problems of Native Americans</li>
<li>MS Office proficiency</li>
<li>Bilingual- Spanish desired</li>
</ul>
<div></div><div><b>PHYSICAL DEMANDS:</b></div>
<ul><li>Able to sit, stand and walk with/ without accommodations.</li>
</ul><div><b>NATIVE AMERICAN PREFERENCE</b>: Preference is given to qualified Native American Applicants in accordance with the Indian Preference Act. If claiming a preference, a copy of valid documentation will be necessary.</div>

<div></div><div><b>FAIR LABOR STANDARDS ACT</b><b>:
</b></div><div>This position is considered to be Non-Exempt and entitled to overtime pay for hours worked in excess of forty (40) hours per workweek for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws.</div></div></div></div>
