<div><div>NeoPath Health, Inc., a Minnesota based administrator of employer based medical services, is in search of a family medicine mid-level practitioner to work at the Prairie Island Health Center in Welch, MN.<br>

<b>Position Summary</b><br>
The Family Practice Physician is an integral part of the NeoPath team. The Family Practice Physician is responsible for providing acute, chronic and preventive medical care in a Primary Care setting.The Family Practice Physician will provide daily oversight of the clinical staff to ensure that the clinic runs in an efficient manner with patient experience as the focus of care.The ideal candidate will be able to build rapport with patients to optimize patient engagement.The individual who holds this position exemplifies NeoPath&rsquo;s commitment to working as part of a highly engaged clinical team to provide an exceptional patient care experience.<br>

<b>Principal Duties and Responsibilities</b>
<ul><li>Provide primary care to patients of all ages including; assessment, diagnosis/treatment of illness/injury, ordering and interpreting medical diagnostic labs and procedures, chronic disease management and wellness care, prescribing of medications, treatments, therapies, medical equipment, procedures, etc.</li></ul>
<ul><li>Evaluate and treat patients in 30 or 60 minutes as scheduled, in person or via remote methods such as virtual visits</li></ul>
<ul><li>Follow up with ordered labs and imaging studies in a timely manner.</li></ul>
<ul><li>Develop treatment plans in collaboration with patients; educate and advise on medical issues.</li></ul>
<ul><li>Educate patients and families on self-care to ensure positive long-term patient outcomes.</li></ul>
<ul><li>Oversight of onsite CLIA waived lab.</li></ul>
<ul><li>Dispense prepackaged medications onsite.</li></ul>
<ul><li>Document all patient interactions in a timely manner.</li></ul>
<ul><li>Present on various related health topics upon request.</li></ul>
<ul><li>Provide clinical leadership.</li></ul>
<ul><li>Function independently, within the scope of practice delineated in the practice agreement.</li></ul>
<ul><li>Maintain appropriate licensure and continuing education requirements.</li></ul><br>

<b>Qualifications</b>
<ul><li>Graduate of a recognized college of medicine. (New grads welcome to apply)</li>
<li>MD license in the state where employment will occur.</li>
<li>Board certification in family medicine preferred.</li>
<li>Active, unrestricted DEA with prescriptive authority in the state where employment will occur.</li>
<li>Minimum of three years of clinical experience in a family practice and/or urgent care setting preferred.</li>
<li>EHR experience required</li>
<li>Ability to cooperate, collaborate and communicate with all levels of the organization.</li>
</ul></div><div>Full-time, working Mondays through Thursdays from 730a-530p. No weekends, evenings, holidays, or after-hours call.<br>
