New Horizon Counseling Center

<div>The Children's Care Coordinator (CCC) functions as a member of an interdisciplinary team to provide support to families and children with serious emotional disturbance, chronic conditions, or trauma experiences. CCC helps the children develop the highest level of functioning and wellness by assisting them to identify strengths and needs, and connecting them to resources in their community.
Essential functions include:
<ul><li>Ongoing assessment, evaluation and ensuring the provision of service needs for each case.</li><li>
Coordination and monitoring of all services inclusive of medical, mental health, and educational wellness/success through collaborative relationships.</li><li>
Home and field visits to provide counseling, crisis intervention, referral and advocacy services, coordination of services, and assistance with basic needs.</li><li>
Work collaboratively with each child and family to develop written care plans and crisis plans, as needed.</li><li>
Preparation of all charting, assessment forms, accountability forms and direct service requirements of state and city regulations.</li><li>
Attend regular supervisory and case conference sessions, trainings and other meetings, as assigned.</li></ul>
What You Need
<ul><li>Bachelor's Degree Required; Masters Degree Preferred in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Human Services, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational therapy. Degrees in other related areas may be considered.</li><li>
Training and successful certification as a CANS-NY Assessor needed within 30 days of start date.</li></ul>
Skills and/or Experience Required:
<ul><li>For B.A. level candidates, two (2) years OR for M.A. level candidates, one (1) year of related human services experience required in providing direct services or linking children with serious emotional disturbances to services.</li><li>
Excellent interpersonal, communication and time management skills, along with English-language writing skills necessary to fulfill state and city regulations for record keeping.</li><li>
Team player with creativity, commitment, and initiative to be a part a growing, dynamic program</li><li>
Ability and willingness to regularly travel, in some instances with clients in agency vehicle to many locations using various modes of reliable and safe transportation</li></ul>
Preferred:
<ul><li>Knowledge of Community Resources, Medicaid, and other entitlements</li><li>
Knowledge of Trauma, Child welfare and/or Child development</li><li>
Bilingual a plus</li></ul>
Job Type: Full Time; Pay Range: Salaries start at $49,000 annually</div>

Our Assertive Community Treatment (ACT) program provides comprehensive, person-centered services designed to support all aspects of a client's life&mdash;including employment and education.
Position Overview 
We are seeking a motivated and compassionate Vocational Specialist to join our Assertive Community Treatment (ACT) Team. In this role, you will support individuals with serious mental illness in obtaining and maintaining competitive employment and pursuing educational opportunities. Working as part of a multidisciplinary team, you will deliver individualized, community-based vocational services aligned with recovery goals and client preferences.
Key Responsibilities
<ul><li>Provide individualized vocational assessment and service planning based on client strengths, interests, and goals</li><li>
Assist clients in job search activities, including resume development, applications, and interview preparation</li><li>
Support clients in accessing educational and vocational training opportunities</li><li>
Integrate employment services with mental health treatment in collaboration with ACT team members</li><li>
Educate clients on workplace expectations, communication skills, and job retention strategies</li><li>
Maintain accurate documentation of services and client progress in accordance with agency standards</li><li>
Participate in daily team meetings, case conferences, and coordinated care activities</li><li>
Provide outreach and engagement services in community-based settings</li><li>
Other duties as assigned</li></ul>
Qualifications
<ul><li>Bachelor's degree in Rehabilitation Counseling, Social Work, Psychology, or a related field preferred (Master's preferred)</li><li>
Minimum of 1&ndash;2 years of experience in vocational services, supported employment, or behavioral health</li><li>
Knowledge of supported employment models (e.g., Individual Placement and Support &ndash; IPS) preferred</li><li>
Strong interpersonal, networking, and organizational skills</li><li>
Ability to engage clients and employers effectively</li><li>
Ability to work independently and collaboratively within a multidisciplinary team</li><li>
Valid driver's license and reliable transportation required</li></ul>
Preferred Qualifications
<ul><li>Experience working in ACT or other community-based behavioral health programs</li><li>
Bilingual skills are a plus</li><li>
Familiarity with electronic health records (EHR) systems</li></ul>
