<div>Program: Medication Assisted Treatment (MAT)

Job Type: Full-Time (40 hours per week)

Days: Monday through Friday, 8am to 5pm, with a 1-hour lunch break

Works with the following population: Males and females, 18 years of age or older, with a mental health disorder and/or addictive diseases.

Salary: $33,072 per year 
 
Under general supervision, the candidate will do the following:
<ul><li>
Receive and follow up on referrals.</li><li>
Make appointments for intake.<ul><li>
After intake, complete referral to New Horizons Behavioral Health MAT-SOR Program.</li></ul></li><li>
Complete Medication Pharmacy Grant Rx Payment Voucher to Genoa Healthcare.</li><li>
Complete DHP Authorization for Release of Protected Health Information (PHI).</li><li>
Complete Midtown Recovery Release of PHI</li><li>
Contact MAT individuals weekly with documentation into WITS system</li><li>
Assist with any needs for MAT, doctor's appointments, getting medication, and assistance with receiving treatment.</li><li>
Coordinate with provider and nurse to address any concerns they may have.</li><li>
Attend meetings for MAT to update on progress.</li><li>
Procure and sign out Narcan.</li><li>
Conduct evidence-based group with individuals who have been identified with a substance abuse disorder.</li><li>
Document daily notes on progress in groups and provide resources in the surrounding community that aid in maintaining sobriety.</li><li>
Aid in increasing coping and basic living skills.</li></ul>

Documentation and Reporting: Maintain accurate confidential records of client interactions and progress. Prepare reports and documentation in compliance with organizational and regulatory requirements.

Collaboration: Collaborate with a multidisciplinary team, including mental health professionals, to ensure holistic care for clients. Attend team meetings and case conferences to contribute a peer perspective to the overall care plan.

Minimum Qualifications:

High School Diploma or GED, with 3 to 5 years of experience in behavioral health -or- completion of a bachelor's degree in the social sciences (Social work, psychology, sociology, counseling, or a related discipline) with 1 year of full-time experience working with individuals with mental health disorders and/or addictive diseases. If candidate has a Bachelors, it MUST be in the above-named fields to be MHC eligible. The candidate MUST have 2 years or more in recovery himself / herself, as lived experience will be used to support individuals in services.

Preferred Qualifications:

A bachelor's degree and experience in working with individuals with mental disorders and/or addictive diseases is preferred.
