CNA/Medical Assistant- Chronic Care Management</b><br>
<b>Location:</b> Bohemia, NY</p><p><b>
Schedule: </b>5/8.5 Hour Shifts M-F<br>
<b>Organization:</b> New York Cancer &amp; Blood Specialists (NYCBS)</p><h3 class="jobSectionHeader"><b>
In This Role, You Will:</b></h3><ul><li><p>
Work within our Chronic Care Management team.</p></li><li><p>
Be responsible for introducing the CCM program to eligible patients via telephone, obtain verbal consent and provide appropriate documentation of this in the EMR system</p></li><li><p>
Under direct supervision of the CCM Supervisor, review and monitor documents related to the patient&rsquo;s individualized care plan.</p></li><li><p>
Advocate for patients, collaborate with other wellness services at NYCBS and provide educational information.</p></li><li><p>
Facilitate the transmission of information between providers and patients, as well as access to services needed.</p></li><li><p>
Document all activities in the EMR system.</p></li></ul><p></p><h3 class="jobSectionHeader"><b>
We Require:</b></h3><ul><li><p><b>
High School Diploma or equivalent.</b></p></li><li><p><b>
Certified CNA, or Medical Assistant</b></p></li><li><p><b>
2+ years of applicable experience.</b></p></li><li><p>
Bilingual, English/Spanish a plus.</p></li><li><p>
Proficiency in Word/Excel.</p></li></ul><p></p><h3 class="jobSectionHeader"><b>

Salary: Starting at $22 per hour based on experience</b></p></li><li><p><b>
