<div><div><div>The Center for Health Equity &amp; Community Wellness (CHECW) seeks to eliminate inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address health gaps and improve health outcomes for all New Yorkers. The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer.<br>

The Bureau of Bronx Neighborhood works with clinical and community partners to expand access to high-quality, culturally responsive programs that prevent and manage chronic diseases, especially for residents facing the greatest health challenges. The Bureau provides public health and social services, coordinates community events, builds collaborative partnerships, and leads research and evaluation initiatives that drive evidence-based action, close health gaps and advance equity across the Bronx. From diabetes and asthma education to nutrition, food security, newborn safety, and childbirth classes, the Bureau empowers New Yorkers with the knowledge and tools to live healthier lives.

The Bureau of Bronx Neighborhood Health is seeking to hire two (2) Clinical &amp; Community Partnerships Facilitators to support the Bronx Asthma Program.
<br>
<br>
DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
<br>
<ul><li>Manage a caseload of 20-30 practices in the engagement of quality improvement initiatives using practice facilitation strategies.
</li><li>Conduct on-site or remote visits to provide technical assistance to practices improving patient health outcomes for asthma.
</li><li>Provide support for workflow redesign and EHR documentation to improve office efficiency for care management.
</li><li>Provide coaching on preventive-health features of EHRs and how to use them as part of QI activities.
</li><li>Serve as a liaison between primary care practices and self-management programs for chronic illness.
</li><li>Act as a content expert for evidence-based intervention (EBI) referral workflow development in small practices.
</li><li>Participate in forums discussing performance feedback and sharing best practices for QI efforts and practice transformation programs.
</li><li>Serve as a liaison between practice staff and various BEHS teams to provide feedback on QI efforts.
</li><li>Deliver presentations and trainings to internal and external audiences.
</li></ul><br>
PREFERRED SKILLS:
<br>
<ul><li>Commitment to social justice and equity.
</li><li>Experience working in low-income communities desired; comfortable working in a community setting with diverse populations.
</li><li>Two plus (2+) years of project management experience.
</li><li>Experience completing or leading quality improvement initiatives in a clinical setting.
</li><li>Knowledge of the Patient Centered Medical Home (PCMH) and Quality Payment Program (QPP) initiatives.
</li><li>Outpatient primary care experience and familiarity with asthma or other chronic diseases.
</li><li>Ability to work collaboratively with physicians, staff, and external organizations.
</li><li>Strong computer skills.
</li><li>Electronic health record and training experience.
</li><li>Excellent time management and communication skill.
</li><li>Excellent skills in presentation, organization, and written and oral communication.
</li><li>Proficient with Microsoft Office suite, including Outlook, Word, Excel, and PowerPoint.</li></ul>
