Registered Nurse (RN ) Job Summary: 
Registered Nurse ( RN ) Major Duties and Critical Tasks: 
<ul>
<li>Works using the guidelines established by the Nurse Practice Act, facility Policies and Procedures, and sound nursing judgment.</li>
<li>Assesses, plans and evaluates nursing care delivered to patients/residents requiring long-term and/or rehabilitation care.</li>
<li>Delivers nursing care to patients/residents requiring long-term and/or rehabilitative care .</li>
<li>Implements the patient/resident plan of care and evaluates the patient/resident response.</li>
<li>Directs and supervises care provided by other nursing personnel.</li>
<li>Provides input in the formulation and evaluation of standards of care.</li>
<li>Maintains knowledge of necessary documentation requirements .</li>
<li>Maintains knowledge of equipment set-up, maintenance and use (i.e. monitors, infusion devices, drain devices, etc.).</li>
<li>Maintains confidentiality and compliance with patient/resident rights, relating to all patient/resident/personnel information.</li>
<li>Provides patient/resident/family/caregiver education as appropriate and/or directed.</li>
<li>Conducts self in a professional manner in compliance with unit and facility policies .</li>
<li>Works rotating shifts, holidays and weekends as scheduled.</li>
</ul>
Registered Nurse ( RN ) Required Education and Experience: 
<ul>
<li>Must be a graduate of a basic education program in professional nursing.</li>
<li>A minimum of one year medical/surgical nursing experience in an acute care (hospital) setting or long-term care setting (acute/critical care is preferred).</li>
<li>Must possess a current Registered Nurse ( RN ) license in state(s) of practice. Must have a reliable source of transportation.</li>
</ul>
The Registered Nurse ( RN ) is responsible to plan and deliver nursing care to patients/residents requiring long-term and/or rehabilitative care.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:
 
<ul>
<li>Admissions, Discharge and Transfers of residents</li>
<li>Medication requisitions with pharmacy and physicians</li>
<li>Medication administration</li>
<li>Following infection control, emergencies, and other center policies</li>
<li>Collaborating with resident’s interdisciplinary team to provide the best outcomes</li>
<li>Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy</li>
<li>Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.</li>
<li>Delivers nursing care to patients/residents requiring long-term or rehabilitative care.</li>
<li>Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.</li>
<li>According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.</li>
<li>In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.</li>
<li>Maintains knowledge of necessary documentation requirements.</li>
<li>Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.</li>

Activities Aide ( CNA ) Job Summary: 
The primary responsibility of the Activities Aide ( CNA ) is to assist the Activity Director in planning, organizing, and conducting activities and programs for the residents that will contribute to their fuller and richer quality of life and to help maintain or increase resident’s ability to meet their living requirements in accordance with the facility mission philosophy and as directed by the Activity Director. As the Activity Assistant, you are delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties.
 
Activities Aide ( CNA ) Major Duties and Critical Tasks: 
<ul>
<li>Assist in planning, developing, organizing, implementing, and evaluating the activity programs of this facility.</li>
<li>Assist in developing and implementing guidelines for the identification of the medically related activity needs of the resident.</li>
<li>Participate in community planning interests that will enhance the mission of the facility and the services and needs of the resident and family.</li>
<li>Participate in discharge planning, development and implementation of activity care plans and resident assessments.</li>
<li>Perform administrative requirements, such as completing necessary forms and reports and submitting them to the Activity Director as required.</li>
<li>Provide consultation to members of the staff, community agencies and others in the process of solving the needs and problems of the resident through the development of activity programs.</li>
</ul>
Activities Aide ( CNA ) Education and Experience: 
<ul>
<li>Certified Nursing Assistant ( CNA ) Certification by the State Required</li>
<li>High School Diploma or GED equivalent.</li>
<li>Skilled nursing experience preferred</li>
<li>Excellent Interpersonal Skills</li>
</ul>
