<div>Position Summary

The Junior Nurse Tech in clinical practice fulfills professional responsibilities regardless of the setting in which care is provided. The JNT functions under the supervision of the LPN Preceptor to assist in coordination of nursing care for the patients by participating in the assessment, nursing diagnosis, and patient care plans. The JNT works cooperatively with ancillary nursing and other patient care team personnel in maintaining standards for nursing practice in the clinical setting. The JNT is an interim position between graduation from an accredited practical nursing program and employment as a LPN. The JNT must take the licensing exam within 3 months of employment as a JNT. Should a JNT not successfully pass the licensing exam they may work as a nurse aide until passing the exam. 


Position Accountabilities

Maintains established departmental policies, procedures, objectives, quality assurance program, safety, environmental and infection control standards.

Under the supervision of the LPN Preceptor, the JNT will contribute to the assessment of the patient&rsquo;s health status and communicate pertinent information and reports changes to appropriate health care personnel.

Under the supervision of the LPN Preceptor, the JNT utilizes the nursing process to provide direct patient care from infancy through geriatrics, evaluates outcome, consults with providers, nursing personnel and other members of the multi-disciplinary team as required and adjusts nursing care processes as indicated to ensure optimal patient care.

Under the supervision of the LPN Preceptor, the JNT performs basic nursing procedures.

Under the supervision of the LPN Preceptor, the JNT maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards.

Under the supervision of the LPN Preceptor, the JNT assists physicians with examinations, procedures and other processes related to direct patient care.

Under the supervision of the LPN Preceptor, the JNT maintains patient&rsquo;s medical records to ensure an accurate record of the patient&rsquo;s medical treatment.

Cooperates with other personnel to achieve department objectives and maintain good employee relations.

Demonstrates sensitivity to patient&rsquo;s comfort and privacy and expresses interest in their progress.

Under the supervision of the LPN Preceptor, the JNT monitors emotional/ physical stability of patients and reports changes to the preceptor, charge nurse, or physician.

Under the supervision of the LPN Preceptor, the JNT is expected to engage in professional role activities appropriate to educational background and is accountable to self, patients, LPN Preceptor and peers for professional actions.

Follows policies and procedures regarding attendance and work hours.

Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.

Under the supervision of the LPN Preceptor, the JNT provides health care instruction to patient and family members.

Under the supervision of the LPN Preceptor, the JNT assists with patient discharge planning.

Attends meetings as required.

Performs other related duties as assigned or requested.
Minimum Education - Post High School Vocational/Specialized Training

Minimum Experience - Sufficient previous clinical experience in areas of assignment

Required Certification/Registration - BLS certification; ACLS; PALS.

Preferred Experience - Previous patient care tech experience

Junior Nurse Tech MED/SURG - Full Time

Position Summary
 
Is accountable for the delivery of transitional care coordination and administrative management of the social services department on a 24 hour basis. Provides oversight to ensure patients receive initial assessment and ongoing evaluations. Facilitates care of patient by insuring that admissions and continued days are medically necessary, cost-effective, and provided in the appropriate level of care to qualify for reimbursement.
 
Position Accountabilities
 
Manages employee(s) providing oversight of task completion for transitional care coordination allocating resources to meet patient/family needs to optimize successful transition from hospital setting.Maintains established departmental policies and procedures, performance improvement projects, safety, environmental and infection control standards.Assists in the development of department goals and objectives, established and implements policies and procedures for department operation.Develops and recommends department operating budget and insures department operations within budget. Provides oversight and management of intervention, both pre-hospital and in-hospital, for patients and families identified from defined screening criteria, as well as through referrals from members of the health care team. Performs oversight and management of activities of assessment, coordination, planning, monitoring, implementation, and evaluation as appropriate.Maintains oversight and/or completion of clear and timely communication with the health care team, including documentation in the medical record, to facilitate comprehensive, efficient and appropriate discharge. Conducts oversight and/or completion of continued stay reviews prior to outside review to determine medical necessity and appropriateness in accordance with established criteria, including, statistical response of completeness compliance. Keeps the Director informed of problematic cases, especially those involving legal or risk management issues. Maintains confidentiality of all patient information. Ensures optimal reimbursement for fee-for-service patients through detailed chart review.Performs concurrent review of patient charts to ensure proper resource utilization and employee education. Reviews performance of employee(s) providing constructive and supportive guidance to improve the quality of their individual performance. Knowledgeable about current rules & regulations pertinent to areas of responsibility.Facilitate appropriate resource utilization.Understand benefits systems & cost benefits analysis, managed care contracts.Maintain knowledge of case management concepts.Prepares reports & compile statistical data on monthly and/or quarterly basis as directed. (Example) 30 day admissions. Referral logs, Medicare message completion compliance. Requests diagnoses from physicians when not recorded on discharge or if information is incomplete.Serves as principal liaison between medical staff, administration, utilization review, nursing, finance, medical records, and other ancillary operational departments with respect to DRG’s and the resolution of technical issues related to participation in the DRG program.Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops.Implements and adheres to policies which ensure patient confidentiality.Assists Physician Committee and subcommittees in identifying areas of misutilization, coordinating with the Director of Performance Improvement.Attends meetings as required, and participates on committees as directed.Performs other duties as assigned.
 
Minimum Education- Associate Degree
 
Nursing Education Preference - Bachelor’s Degree
 
Required Licensure/Certifications - Current licensure as an LPN or RN in the State of Oklahoma Basic Life Support
 
Certification
 
Preferred Licensure/Certifications - Case Management certification
 
Experience - Three to five years of inpatient/hospital experience.
 
Preferred Experience - Case management, discharge planning or utilization management experience

Discharge Planning/Case Management - Full Time

Position Summary
 
Provides basic nursing care to patients under direct supervision. Position previously called Certified Nurse Aide, Nurse Aide, and Critical Care Tech.
 
Position Accountabilities
 
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards. Consistently follows established protocols and procedures.
 
Performs basic nursing procedures.
 
Obtains and documents patient vital signs, including temperature, blood pressure, weight, height, respiration and pulse.
 
Provides appropriate, direct patient care and personal care assistance to patients, including, but not limited to, bathing, oral hygiene, dressing, eating, toileting and providing transport.
 
Assists patients with activities of daily living, encouraging independent behavior when appropriate.
 
Prepares patients for scheduled procedures to assist in patient assessment.
 
Monitors medical condition of patients and reports changes to appropriate personnel.
 
Demonstrates sensitivity to patient’s comfort and privacy and expresses interest in their progress.
 
Maintains patient records and charts to ensure an accurate record of the patient’s medical treatment.
 
Maintains confidentiality of patient and unit care data.
 
Cooperates with other personnel to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection control policies.
 
Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards.
 
Cleans and prepares rooms between procedures and after terminal cleaning. Monitors and restocks supplies.
 
Orients, instructs, and trains assigned personnel. Communicates technical information in a manner appropriate for intended audience. Serves as a resource for co-workers.
 
Follows policies and procedures regarding attendance and work hours. Demonstrates flexibility in working hours and days scheduled in order to meet the needs of the hospital. Works scheduled shifts. Reports to work on time. Attends required number of staff meetings per year.
 
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
 
Performs other related duties as assigned or requested.
 
Minimum Education - Less than High School
 
Required Certification/Registration - BLS certification
 
Preferred Education - High school or equivalent
 
Preferred Experience - Previous clinical experience
 
Preferred Course(s) / Training - Completion of course for nursing assistants.
