Our Residential Case Manager will work out of our Albany, NY campus and will be responsible for supporting clients in the Residential System while performing duties in collaboration with a clinician.
</p><p></p><p><b>ESSENTIAL DUTIES AND RESPONSIBILITIES</b></p><ul><li>
Arrange and attend home visits</li><li>
Prepare transition plans</li><li>
Coordinate Independent Living Services</li><li>
Act as the primary contact between Schools and Residential</li><li>
Coach family visits</li><li>
Complete various assessments, including FASP and SPR</li><li>
Complete accurate case notes and timely input the notes into Connections database</li><li>
Provide transportation for clients</li><li>
Maintain contact with DSS and family</li><li>
Function as a contributing member of the interdisciplinary treatment team</li><li>
Attend dorm meetings</li><li>
Attend treatment review meetings</li></ul><h1 class="jobSectionHeader"><b>
</b></h1><h1 class="jobSectionHeader"><b>EDUCATION and/or EXPERIENCE
</b></h1><p>Requires a Bachelors degree or commensurate level of education and related experience.</p><p>
Experience with individuals in care systems preferred.
</p><p><b>Drivers license is required for this position.</b></p><p>
Work requires 25 50% community travel/visits.</p><p>
