Summary:
The Human Resources Business Partner (HRBP) is responsible for delivering exceptional HR services across defined organizational unit(s) in alignment with organizational objectives. Serves as an extended leadership team member to management, actively engages in the facilitation of solutions to business challenges, and influences positive change. Increases organizational effectiveness and delivers solutions that retain and develop talent. Ensures management and employees have the support and guidance they need in order to be successful.
Responsibilities:
1. Assesses, evaluates, and identifies areas for management development/improvement to optimize the current and future operational performance of the unit, including influencing positive outcomes. Coaches managers to encourage ongoing expansion of management capabilities, collaboration, change management, and competency of critical healthcare leader attributes. 
2. Leverages annual processes to identify opportunities to improve organizational structures, address competency and performance gaps, support the development of top talent, and plan for the future. Partners with management to maximize the completion of annual and ongoing performance management, compensation planning, and succession planning. 
3. Understands the unit and organization&iuml;&iquest;&frac12;s business strategies and challenges, and applies human resources strategies and programs to facilitate achievement of the unit&iuml;&iquest;&frac12;s business goals. 
4. Implements best in class human resources planning in alignment with employer of choice initiatives. Develops and delivers against comprehensive people plans, which translate the strategic and tactical business plans into HR operational plans. 
5. Actively participates in the sourcing of management roles and the selection of exceptional talent. 
6. Oversees a highly effective, fair, and consistent employee relations program for the assigned unit. Partners with Employee Relations resources to ensure that issues are addressed in a timely manner through a comprehensive, thorough, and objective investigation and resolution process. Coaches managers on the effective resolution of conflict, and how to foster a positive working environment. 
7. Manages relationships with union representatives to provide proactive resolution of issues. Ensures that labor contract provisions are applied accurately and consistently. Serves as a key organizational representative at Labor Management meetings.
 
8. Identifies needs within the assigned unit and partners with HR Leadership to develop and deliver programs including training, development, competitive pay, and creative staffing. 
9. Analyzes and works with data to understand positive and negative drivers of business issues, employee engagement, retention, and diversity. Proposes plans to address current and future challenges in response to data analysis. 
10. Fosters open and transparent organizational communication, ensuring that key business and human resources information is widely shared in a format and method focused on the needs of the employee. 
11. Participates in system-wide projects and/or processes that directly impact the ability to deliver exceptional service at all levels of the organization. 
12. Fulfills all compliance responsibilities related to the position. Identifies opportunities for policy improvements.

Education: BACHELOR'S LVL DGRE
Other Information:<ul><li>
This position requires a minimum formal education of Bachelor Degree and minimum of 7-10 years of experience as an HR Generalist.</li><li>
Working knowledge of multiple human resources disciplines including compensation, organizational design, management development, employee and labor relations, performance management, diversity, and federal and state employment laws.</li><li>
Demonstrated creative problem solving skills, with a high degree of business acumen and the ability to quickly grasp complex business issues.</li><li>
Adept at working with management to influence/facilitate change and the achievement of organizational objectives.</li><li>
Exceptional interpersonal, analytical, and organizational/presentation skills, as well as verbal and written communication skills.</li><li>
Experience with human resources information systems, including report management.</li><li>
Masters degree in human resources, business, organizational development, industrial/organizational psychology or other related discipline preferred.</li></ul>
License, Registration, Certification Requirements: None required. SHRM-CP, SHRM-SCP, HRCI SPHR, or HRCI PHR preferred.

HR Business Partner

<div>Description
Summary: 
Under the direct supervision of the Manager of Ambulatory Care Management, the care manager, registered nurse (RN) serves in an expanded nursing role to coordinate and provide care management services to patients at all risk levels. The care manager (RN) collaborates with primary care providers in multiple practice sites to facilitate clinical guideline adherence and to coordinate services specifically related to patients' healthcare needs. The care manager (RN) integrates identified needs of patient, education pathways, clinical guidelines, and diagnostic tests to develop individualized plans of care.
Responsibilities: 
1. Assesses, plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the patient's health status. Assesses healthcare, educational and psychosocial needs of the patient and their family.
2. Integrates evidenced-based clinical and preventative guidelines, protocols and other metrics to develop treatment plans that are patient-centric. Promote quality and efficiency in the delivery of healthcare for the population served. Uses established education pathways to facilitate comprehensive and consistent condition management education to patients.
3. Assesses patient at the point of disposition to optimize patient centric plan of care. Identifies the targeted population and stratifies risk of all patients to prioritize needs and direct interventions. Ability to provide Transitions of Care.
4. Designs an individualized plan of care with the patient and fosters a team approach by working collaboratively with the patient, family, primary care provider, and other members of the healthcare team to ensure coordination of services.
5. Demonstrates ability to work autonomously and be directly accoutable for results. Collaborates with primary care providers to enhance evidence-based clinical guideline adherence and promote best practices by initiating/adjusting therapies as directed by the provider providing appropriate follow-up and monitoring as needed. Facilitates/coordinates laboratory and diagnostic tests for all condition management programs.
6. Evaluates patients' progress toward desired heatlh outcomes and adjusts plan of care for optimal outcomes.
7. Collects, documents and analyzes data (pre-visit planning) communicating results/documents as required for care management programs.
8. Maintains patient confidentiality with all activities in the care management programs.
9. Identifies problems, proposes process improvement strategies, and assists the care management team in quality improvement activities.
10. Collaborates with other disciplines at practice sites to identify operational issues and performance improvement needs.
11. Attends and participates in professional and nursing continuing education programs as needed to support the care management programs and professional development. Reviews current literature on effective teaching/learning strategies and incorporates appropriate techniques into their patient education sessions.
12. Provides support to other members of the care management team
13. Performs other duties as required or assigned by emergency or other operational reasons for which the employee is qualified to perform.
14. Fulfills all compliance responsibilities related to the position.
