<div>What You Should Know About the RN Team Nurse Role:<ul><li>
This is a full-time RN Team Nurse position serving Newark and surrounding areas.</li><li>
Schedule: 8:30am - 6:30pm with weekend and holiday rotation.</li><li>

The RN Team Nurse's Essential Duties Are:
The RN Team Nurse carries the responsibility of flexibility in diverse Ohio&rsquo;s Hospice RN roles as a float team member assisting Care Partners, Coming Home, Extended-Hours/On-Call and the Admission Departments and IPU as needed. Float team staff will collaborate with various IDT members both internal and external of the organization. Clinical services will be scheduled in a timely fashion according to the patient&rsquo;s plan of care, observing patient&rsquo;s continuity of care to achieve the desired outcome of comfortable and peaceful death for patients. When in the field, staff will provide excellent clinical care to all Ohio&rsquo;s Hospice patients that meet the needs of our patients/families, Ohio&rsquo;s Hospice standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
Qualifications:<ul><li>
RN Nurse License in the State of Ohio without any board actions Bachelor Degree preferred</li><li>
Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.</li><li>
CPR Certified</li><li>
Computer Skills sufficient to properly document services and care</li><li>
Pain and Symptom Management Skills</li><li>
Assessment Skills</li><li>
Valid Driver&rsquo;s license in the state of Ohio</li><li>
State minimum auto insurance</li><li>
Safe driving record</li><li>
Must be able to pass a criminal background check</li><li>
Must be able to pass a 10 panel drug screen</li></ul>

<div>What You Should Know About the LPN Crisis Care - Individualized Care Role:<ul><li>
This is a Full Time DRIVING position covering Wooster and surrounding areas</li><li>
Hours are 8p-8a 3 days a week, with every other weekend and holiday requirements</li><li>
We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed</li></ul>
The LPN Crisis Care Essential Duties Are:
We provide superior care for patients in families in homes/facilities. The Crisis Care LPN will provide quality nursing care to patients in a manner that meets the patient and family needs, working under the direction and supervision of an RN. You will provide superior care to patients and their families.
LPN Individualized Care Qualifications:<ul><li>
Registered LPN with the State of Ohio without any board actions.</li><li>
One year of nursing experience preferred.</li><li>
CPR Certified.</li><li>
Computer skills sufficient to properly document services and care.</li><li>
Ability to drive during daytime, nighttime or inclement weather.</li><li>
Valid Driver's License with Safe Driving Record.</li><li>
State Minimum Automobile Insurance Coverage.</li><li>
Must be able to pass a criminal background check</li><li>
Must be able to pass a 10 screen drug panel</li></ul>
