Case Manager I/II
Annual Salary:<ul><li>
Level I H21A - $ 40,000.87 + Full State Employee Benefits</li><li>
Level II H21B - $43,000.94 + Full State Employee Benefits</li></ul>
Travel is extensive. Must possess a valid driver's license and must maintain required car insurance. 
Minimum Qualifications:
All New Employees will complete the Case Manager Academy Training Program
Case Manager - Level I<ul><li>
Education and Experience requirements at this level consist of a bachelor&rsquo;s degree in a human services field* and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
</li><li>OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.
</li><li>For purposes of the Case Manager job family &ldquo;a bachelor&rsquo;s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.</li></ul> 
Case Manager - Level II<ul><li>
Education and Experience requirements at this level consist of a bachelor&rsquo;s degree in a human services field* and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field;
</li><li>OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.

Job Responsibilities:<ul><li>
Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives; obtains in-depth social histories, develops and assures community and/or agency resources to meet the consumer's needs; serves as QIDP and provides monthly monitoring of the program.</li><li>
This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.</li></ul>
