On My Watch case management&rsquo;s goal is to connect provider and community resources to enable clients to manage disease, remain in their communities, and prevent premature admissions to nursing facilities.On My Watch, Inc.Elderly and Disabled Waiver Program - SOURCE/EDWPSocial Worker/Case Manager needed in the Atlanta Regional Commission Area of Division of Aging Services.Main Office located in Athens, GASatellite Offices: Augusta, GA and Savannah, GA General Description: Full time social worker position available for the Elderly and Disabled Medicaid Waiver Program. Under direction, performs work of moderate difficulty by providing skilled casework services to selected caseloads or clients with special problems such as health disability or those at risk of nursing home placement; provides specialized casework services aimed at securing the client's overall well-being and maximum degree of independent functioning. Serves large geographic areas which may include one large county and/or many small counties which may involve extensive travel, and performs related work as required.Qualification: A. Minimum Education and Experience *Bachelor's degree in social work, sociology, psychology, or a related field, AND *Two years experience in the human service or health related field. B.
